INDICATION

Aflor Eye Drops 5ml is an ophthalmic corticosteroid indicated for the treatment of inflammatory conditions of the palpebral and bulbar conjunctiva, cornea, and anterior segment of the globe where the risk of steroid use is accepted to obtain a reduction in oedema and inflammation. It is used for allergic conjunctivitis, vernal conjunctivitis, uveitis affecting the anterior segment, episcleritis, and post-operative inflammatory conditions following ocular surgery such as cataract extraction. It is preferred over potent ophthalmic corticosteroids in patients at risk of elevated intraocular pressure.

MECHANISM OF ACTION

Fluorometholone is a synthetic corticosteroid that binds to glucocorticoid receptors in ocular tissue, modulating the transcription of anti-inflammatory genes and suppressing the production of prostaglandins, leukotrienes, and pro-inflammatory cytokines in the eye. It inhibits phospholipase A2, preventing arachidonic acid release and subsequent inflammatory mediator production. Fluorometholone has a lower tendency to raise intraocular pressure compared to prednisolone or dexamethasone because it is more rapidly metabolized in the trabecular meshwork and has lower glucocorticoid receptor affinity in this tissue, making it a preferred corticosteroid in patients susceptible to steroid-induced glaucoma.

DOSAGE AND ADMINISTRATION

•      Instil 1 to 2 drops into the affected eye(s) 2 to 4 times daily

•      During the initial 24 to 48 hours, dosing may be increased to every 4 hours if necessary

•      Shake the bottle well before each use as this is a suspension

•      Do not use for more than 14 days without reassessment by an ophthalmologist; always follow your doctor's instructions

SIDE EFFECTS

Common:

•      Transient stinging or burning on instillation

•      Temporary blurring of vision after instillation

•      Eye irritation or foreign body sensation

•      Mild conjunctival hyperemia

Less common:

•      Elevated intraocular pressure (steroid-induced glaucoma) with prolonged use; monitor IOP

•      Posterior subcapsular cataracts with prolonged use

•      Secondary bacterial, viral, or fungal ocular infection due to immunosuppression

•      Delayed wound healing after ocular surgery

NOT RECOMMENDED IN

•      Epithelial herpes simplex keratitis (dendritic keratitis) and other active viral diseases of the cornea and conjunctiva

•      Mycobacterial ocular infections and fungal diseases of the eye

•      Known hypersensitivity to Fluorometholone or other corticosteroids

•      Do not use with contact lenses in place; remove lenses before instillation and wait 15 minutes before reinserting

DRUG INTERACTIONS

Topical ophthalmic corticosteroids have minimal systemic absorption and no clinically significant systemic drug interactions at standard doses. If concurrent topical antimicrobials are prescribed, Instil the corticosteroid at least 5 minutes after the antibiotic to prevent washout. Concomitant use with non-steroidal ophthalmic drops may reduce anti-inflammatory efficacy.

Store between 2°C and 25°C. Shake well before use. Do not freeze. Protect from direct sunlight. Discard 28 days after opening. Keep out of reach of children.
