INDICATION

BFL Forte Drop is a higher-strength ophthalmic corticosteroid suspension containing fluorometholone 0.25%, indicated for the treatment of moderate to severe inflammatory conditions of the palpebral and bulbar conjunctiva, cornea, and anterior segment where greater anti-inflammatory efficacy is required than the standard 0.1% formulation can provide. It is used in more severe allergic conjunctivitis, anterior uveitis, keratitis, and post-operative ocular inflammation after surgery.

MECHANISM OF ACTION

Fluorometholone at 0.25% provides higher corticosteroid concentration at the ocular surface compared to the 0.1% formulation, producing more potent suppression of the inflammatory cascade in the anterior eye. It binds to glucocorticoid receptors in ocular tissue and modulates transcription of anti-inflammatory genes, reducing prostaglandin, leukotriene, and cytokine production. Its relatively lower tendency to raise intraocular pressure compared to prednisolone or dexamethasone makes it a preferred option even at higher concentrations in patients susceptible to steroid-induced glaucoma.

DOSAGE AND ADMINISTRATION

•      Instil 1 to 2 drops into the affected eye(s) 2 to 4 times daily

•      Shake the bottle well before each use

•      During severe inflammation, the frequency may be increased under ophthalmologist supervision

•      Do not use for more than 14 to 21 days without ophthalmological review; monitor intraocular pressure regularly

SIDE EFFECTS

Common:

•      Transient stinging or burning on instillation

•      Temporary blurring after instillation

•      Eye irritation and foreign body sensation

•      Mild conjunctival hyperemia

Less common:

•      Elevated intraocular pressure (steroid-induced glaucoma) with prolonged use; monitor IOP

•      Posterior subcapsular cataracts with prolonged therapy

•      Secondary bacterial, viral, or fungal ocular infection

•      Delayed corneal wound healing

NOT RECOMMENDED IN

•      Epithelial herpes simplex keratitis and active viral ocular diseases

•      Mycobacterial and fungal ocular infections

•      Known hypersensitivity to fluorometholone

•      Remove contact lenses before instillation; wait 15 minutes before reinserting

DRUG INTERACTIONS

Same as Aflor Eye Drops. No significant systemic interactions. If using concurrent topical ophthalmic medications, Instil them at least 5 minutes apart.

STORAGE INSTRUCTIONS

Store between 2°C and 25°C. Shake well before use. Do not freeze. Protect from direct sunlight. Discard 28 days after opening. Keep out of reach of children.
