INDICATION

Zoloft 100mg Tab is indicated for the treatment of major depressive disorder (MDD), generalized anxiety disorder (GAD), panic disorder with or without agoraphobia, post-traumatic stress disorder (PTSD), obsessive-compulsive disorder (OCD), social anxiety disorder, and premenstrual dysphoric disorder (PMDD) in adults. It is also approved for the treatment of OCD in children and adolescents aged 6 years and older. At the 100mg dose, Zoloft provides a higher level of serotonin reuptake inhibition appropriate for patients who have not achieved adequate symptom control on the standard starting dose of 50mg.

MECHANISM OF ACTION

Sertraline is a selective serotonin reuptake inhibitor (SSRI) that works by blocking the serotonin transporter (SERT) protein on presynaptic neurons, preventing the reabsorption of serotonin from the synaptic cleft. This increases the concentration of serotonin available to bind postsynaptic receptors in brain circuits that regulate mood, anxiety, and emotional processing, including the limbic system and prefrontal cortex. The sustained elevation of synaptic serotonin gradually normalizes the dysregulated neurotransmitter activity underlying depressive and anxiety disorders, producing therapeutic effects that typically develop over 2 to 4 weeks of regular treatment.

DOSAGE AND ADMINISTRATION

•      Standard adult dose: 50mg once daily initially; may be increased to 100mg once daily after a minimum of one week.

•      Maximum approved dose: 200mg once daily.

•      Take orally with or without food, at the same time each day.

•      Swallow whole with water; do not crush or break.

•      Do not discontinue abruptly; taper the dose gradually under physician supervision to prevent discontinuation syndrome.

SIDE EFFECTS

•      Common: Nausea, dry mouth, insomnia, diarrhea, dizziness, increased sweating, fatigue, and sexual dysfunction.

•      Less common: Weight changes, tremor, palpitations, agitation, somnolence.

NOT RECOMMENDED IN

Zoloft 100mg Tab is contraindicated with concurrent or recent (within 14 days) use of monoamine oxidase inhibitors (MAOIs) due to the risk of potentially fatal serotonin syndrome. It must not be co-administered with pimozide or thioridazine due to QT prolongation risk. Contraindicated in patients with known hypersensitivity to sertraline. Use with caution in patients with epilepsy, bipolar disorder, hepatic impairment, bleeding disorders, diabetes, and narrow-angle glaucoma.

DRUG INTERACTIONS

Concurrent use with MAOIs, linezolid, methylene blue, tramadol, triptans, lithium, and other serotonergic agents increases the risk of serotonin syndrome. NSAIDs, aspirin, and anticoagulants combined with sertraline increase gastrointestinal and systemic bleeding risk. CYP2C19 inhibitors such as omeprazole and cimetidine may increase sertraline plasma concentrations. Sertraline inhibits CYP2D6 and may increase plasma levels of tricyclic antidepressants, antipsychotics, and other CYP2D6 substrates.
