INDICATION

Zithrocin 250mg Tab is a macrolide antibiotic indicated for the treatment of mild to moderate bacterial infections in adults and children caused by susceptible organisms. Its indications include community-acquired pneumonia, acute bacterial exacerbations of chronic bronchitis, pharyngitis and tonsillitis caused by Streptococcus pyogenes, uncomplicated skin and soft tissue infections, non-gonococcal urethritis and cervicitis caused by Chlamydia trachomatis, and acute otitis media in children. Its short 3 to 5 day dosing course, once-daily administration, and favorable tissue penetration make it one of the most widely prescribed antibiotics globally.

MECHANISM OF ACTION

Azithromycin is an azalide, a subclass of the macrolide antibiotics. It inhibits bacterial protein synthesis by binding reversibly to the 50S ribosomal subunit of susceptible bacteria, blocking the translocation step of peptide chain elongation and inhibiting mRNA-directed protein production. Without essential proteins, bacteria cannot grow, replicate, or maintain their vital functions, making azithromycin primarily bacteriostatic. DOSAGE AND ADMINISTRATION

•      Community-acquired pneumonia and acute bronchitis: 500mg on day one, then 250mg once daily on days two to five.

•      Pharyngitis, tonsillitis, and skin infections: 500mg on day one, then 250mg once daily for four days.

•      Chlamydia: 1g as a single dose (four 250mg tablets taken together).

•      Take at least one hour before or two hours after a meal for capsule formulations; tablets may be taken with food.

•      Complete the full prescribed course even if symptoms resolve before the course ends.

SIDE EFFECTS

•      Common: Nausea, diarrhea, abdominal discomfort, vomiting, headache.

•      Less common: Dizziness, skin rash, elevated liver enzymes, photosensitivity.

NOT RECOMMENDED IN

Zithrocin 250mg Tab is contraindicated in patients with known hypersensitivity to azithromycin, erythromycin, or other macrolide antibiotics. It must not be used in patients with pre-existing QT prolongation or those receiving QT-prolonging drugs due to the risk of torsades de pointes. Use with caution in patients with significant hepatic impairment, myasthenia gravis, or a history of antibiotic-associated colitis. Not recommended during pregnancy unless clearly necessary.

DRUG INTERACTIONS

Antacids containing aluminum or magnesium may reduce azithromycin peak plasma concentration; administer separately by at least 2 hours. Concurrent use with warfarin may enhance anticoagulant activity, and INR should be monitored. Co-administration with ergotamine or dihydroergotamine is not recommended due to the risk of acute ergot toxicity. QT-prolonging drugs, including certain antiarrhythmics, antipsychotics, and fluoroquinolones, have additive cardiac effects.
