INDICATION

Zithrocin 500mg Tab is a macrolide antibiotic indicated for the treatment of bacterial infections in adults caused by susceptible organisms. Primary indications include community-acquired pneumonia caused by Streptococcus pneumoniae, Haemophilus influenzae, Mycoplasma pneumoniae, and Chlamydophila pneumoniae; acute bacterial sinusitis; acute exacerbations of chronic obstructive pulmonary disease; pelvic inflammatory disease; and typhoid fever in settings with documented susceptibility. At 500mg, Zithrocin provides the loading dose required for initial tissue saturation in moderate infections and is the standard dose for most adult azithromycin indications.

MECHANISM OF ACTION

Azithromycin exerts its antibacterial effect by binding to the 50S ribosomal subunit of susceptible bacterial organisms, thereby inhibiting RNA-dependent protein synthesis at the translocation step. This prevents bacterial cells from producing the structural and enzymatic proteins necessary for growth, cell wall maintenance, and reproduction. Azithromycin accumulates extensively within phagocytic cells, including macrophages and neutrophils, reaching concentrations at sites of infection that are 10 to 100 times higher than corresponding plasma levels. This intracellular accumulation makes it particularly effective against intracellular pathogens such as Chlamydia, Mycoplasma, and Legionella, and supports its anti-inflammatory and immunomodulatory properties that contribute additional benefit in respiratory tract infections.

DOSAGE AND ADMINISTRATION

•      Community-acquired pneumonia: 500mg once daily for 3 days, or 500mg on day one followed by 250mg once daily on days two to five.

•      Pelvic inflammatory disease: 500mg intravenously once daily for 1 to 2 days, then 250mg orally once daily for 5 to 7 days.

•      Acute sinusitis and COPD exacerbations: 500mg once daily for 3 days.

•      Take with or without food; swallow whole with water.

•      Complete the full prescribed course to prevent antibiotic resistance and recurrence.

SIDE EFFECTS

•      Common: Nausea, diarrhea, abdominal pain, vomiting, headache, dizziness.

•      Less common: Elevated liver transaminases, skin rash, palpitations, fatigue, taste disturbances.

•      Rare: QT interval prolongation and torsades de pointes, severe hepatotoxicity, Stevens-Johnson syndrome, anaphylaxis, Clostridioides difficile colitis.

NOT RECOMMENDED IN

Zithrocin 500mg Tab is contraindicated in patients with hypersensitivity to azithromycin or any macrolide antibiotic, patients with pre-existing QT prolongation or significant bradyarrhythmias, and those with severe hepatic impairment. Use with caution in patients with known cardiac arrhythmias, hypokalemia, hypomagnesemia, and myasthenia gravis. Not recommended for use in children for this indication and strength.

DRUG INTERACTIONS

Concurrent use with QT-prolonging drugs including antiarrhythmics, antipsychotics, and fluoroquinolone antibiotics carries additive cardiac risk. Warfarin anticoagulant effect may be enhanced by azithromycin; INR monitoring is recommended during co-administration. Ergot alkaloids including ergotamine carry risk of acute ergotism and should not be co-administered. Antacids reduce azithromycin absorption and should be taken separately by at least 2 hours.
