INDICATION

Loqathro 500mg is a broad-spectrum macrolide antibiotic indicated for the treatment of mild to moderate infections caused by susceptible organisms. It is commonly used for community-acquired pneumonia, acute exacerbations of chronic bronchitis, pharyngitis, tonsillitis, sinusitis, and otitis media. Loqathro is also effective in treating skin and soft tissue infections, uncomplicated urethritis, and cervicitis caused by Chlamydia trachomatis. Its extended tissue half-life allows for shorter treatment durations, which improves patient compliance.

MECHANISM OF ACTION

Azithromycin inhibits bacterial protein synthesis by binding reversibly to the 50S ribosomal subunit of susceptible microorganisms. This binding blocks translocation of peptides, halting the elongation of bacterial proteins and ultimately inhibiting bacterial growth. Unlike bactericidal agents, azithromycin is primarily bacteriostatic at standard doses. Its unique pharmacokinetic profile allows it to concentrate in tissues and phagocytes, providing prolonged antibacterial activity at infection sites even after the final dose.

DOSAGE AND ADMINISTRATION

•      Adults: 500mg once daily for 3 days, or 500mg on day 1 followed by 250mg on days 2–5.

•      For urogenital infections: Single 1g dose (two 500mg tablets).

•      Take 1 hour before or 2 hours after meals for optimal absorption.

•      Complete the full prescribed course even if symptoms resolve earlier.

•      Dose adjustments required in patients with severe renal or hepatic impairment.

It can elevate plasma levels of warfarin, making regular INR monitoring necessary. Antacids containing aluminum or magnesium may lower the peak concentration of azithromycin and should not be taken at the same time. Concurrent use with ergotamine derivatives may increase the risk of ergotism. Additionally, azithromycin may interact with cyclosporine and enhance its nephrotoxic effects.
