INDICATION

Zibes 5mg Tab is a cardioselective beta-1 adrenoreceptor blocker indicated for the management of hypertension (high blood pressure), stable chronic angina pectoris (chest pain due to coronary artery disease), and stable chronic heart failure with reduced left ventricular ejection fraction as an adjunct to ACE inhibitors and diuretics, with or without cardiac glycosides. At 5mg, Zibes represents the standard initiation dose for most cardiac indications and the effective maintenance dose for mild to moderate hypertension and stable heart failure.

MECHANISM OF ACTION

Bisoprolol fumarate selectively blocks beta-1 adrenergic receptors in the heart, without clinically significant activity at beta-2 receptors in the lungs and peripheral vasculature at therapeutic doses. By blocking the effect of circulating catecholamines (adrenaline and noradrenaline) at cardiac beta-1 receptors, bisoprolol reduces heart rate, decreases myocardial contractility, and lowers cardiac output, resulting in reduced blood pressure and decreased myocardial oxygen demand. In heart failure, the chronic sympathetic overstimulation that accelerates cardiac remodeling is attenuated, improving ventricular function and reducing mortality. In angina, reduced heart rate and contractility decrease myocardial work and oxygen consumption, alleviating ischemic chest pain.

DOSAGE AND ADMINISTRATION

•      Hypertension: Start at 5mg once daily; may increase to 10mg or 20mg once daily as needed.

•      Angina: 5 to 10mg once daily,, depending on symptom control and heart rate response.

•      Stable heart failure: Start at 1.25mg once daily; titrate slowly upward to a maximum of 10mg once daily under specialist supervision.

•      Take once daily in the morning with or without food at the same time each day.

•      Do not stop abruptly; taper gradually under physician supervision as sudden withdrawal may precipitate cardiac events.

SIDE EFFECTS

•      Common: Fatigue, bradycardia (slow heart rate), dizziness, cold hands and feet, headache.

•      Less common: Nausea, diarrhea, constipation, sleep disturbances, depression.

NOT RECOMMENDED IN

Zibes 5mg Tab is contraindicated in cardiogenic shock, decompensated heart failure requiring IV inotropic support, sick sinus syndrome, second or third degree AV block without a pacemaker, severe bradycardia (heart rate below 60 beats per minute before starting treatment), severe peripheral arterial disease, phaeochromocytoma (without concurrent alpha-blockade), metabolic acidosis, and known hypersensitivity to bisoprolol or any beta-blocker. Use with extreme caution in patients with reactive airway disease (asthma or COPD) and in diabetic patients prone to hypoglycemia, as bisoprolol may mask hypoglycemic symptoms.

DRUG INTERACTIONS

Calcium channel blockers of the verapamil and diltiazem class must not be co-administered intravenously with bisoprolol due to the risk of severe bradycardia, AV block, and heart failure. Antiarrhythmic drugs and other beta-blockers have additive effects on heart rate and conduction. Clonidine withdrawal can cause rebound hypertension if bisoprolol is not tapered simultaneously. Insulin and oral antidiabetics may have their hypoglycemic effects prolonged and their symptoms masked. NSAIDs may reduce the antihypertensive effect of bisoprolol. Sympathomimetics have antagonistic effects.
