INDICATION

Zexa 10mg Tab is indicated for the treatment of moderate to severe dementia associated with Alzheimer's disease in adults. It is used to improve or stabilize cognitive symptoms, including memory, attention, reasoning, language, and the ability to perform daily activities. It may also reduce the behavioral disturbances that frequently accompany Alzheimer's disease, including agitation and aggression. Memantine does not cure or halt the underlying disease process but provides meaningful symptomatic benefit and can slow the rate of functional decline in patients with established moderate to severe Alzheimer's disease. MECHANISM OF ACTION

Memantine is a low-to-moderate affinity, uncompetitive NMDA receptor antagonist used in Alzheimer’s disease that attenuates pathological glutamate-mediated receptor overactivation. This persistent overactivity drives excessive calcium influx, leading to excitotoxic neuronal injury. Memantine blocks the receptor in a voltage-dependent manner, preferentially during abnormal activation, while rapidly disengaging under physiological conditions. This preserves normal synaptic transmission. By reducing excitotoxicity and optimizing glutamatergic signaling, memantine supports cognitive function.

DOSAGE AND ADMINISTRATION

Not recommended for use in children for Alzheimer's disease.

DRUG INTERACTIONS

NMDA antagonists, including amantadine, ketamine, and dextromethorphan, should be co-administered with caution as additive NMDA blockade may produce adverse effects, including psychosis and confusion. Alkalinizing agents, including carbonic anhydrase inhibitors and sodium bicarbonate, increase urinary pH and reduce memantine clearance, potentially leading to toxicity. Drugs excreted by the same renal tubular cation transport system, including cimetidine, ranitidine, procainamide, quinidine, and nicotine, may have altered plasma concentrations when co-administered. L-DOPA and dopaminergic agonists may have enhanced effects.
