INDICATION

Xobix 15mg Tab is a preferential COX-2 inhibitor of the oxicam class of NSAIDs, indicated for the symptomatic relief of pain and inflammation in adults and adolescents. Its approved indications include osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and juvenile idiopathic arthritis. It is also used for the short-term management of acute musculoskeletal pain, soft tissue injuries, and post-surgical pain where anti-inflammatory and analgesic coverage is required. At the 15mg dose, Xobix provides maximal anti-inflammatory efficacy and is appropriate for patients with moderate to severe inflammatory arthritis or those whose symptoms are inadequately controlled on the lower 7.5mg dose.

MECHANISM OF ACTION

Meloxicam is a non-steroidal anti-inflammatory drug (NSAID) with preferential COX-2 inhibition. It reduces the production of inflammatory prostaglandins responsible for pain, swelling, and fever, thereby providing effective analgesic and anti-inflammatory effects. Because of its relative selectivity for COX-2 over COX-1, meloxicam is associated with a lower risk of gastrointestinal irritation compared to non-selective NSAIDs like ibuprofen or naproxen. However, at higher doses such as 15 mg, this selectivity decreases, leading to some COX-1 inhibition and a correspondingly increased risk of gastrointestinal side effects.

DOSAGE AND ADMINISTRATION

•      Adult standard dose: 15mg once daily taken orally with a full glass of water.

•      Take with food or milk to reduce the risk of gastrointestinal upset.

•      Remain upright for at least 10 minutes after taking the tablet to reduce esophageal irritation.

•      Use the lowest effective dose for the shortest duration necessary to control symptoms.

•      Do not exceed 15mg per day in adults; higher doses do not improve efficacy and substantially increase the risk of adverse effects.

SIDE EFFECTS

•      Common: Dyspepsia, nausea, abdominal pain, diarrhea, constipation, headache, dizziness.

•      Less common: Fluid retention and peripheral edema, elevated blood pressure, elevated hepatic transaminases, skin rash.

Xobix 15mg Tab is contraindicated in patients with a history of gastrointestinal ulceration, perforation, or bleeding; established ischemic heart disease, peripheral arterial disease, or cerebrovascular disease; severe heart failure; severe renal impairment (eGFR below 30ml/min unless on dialysis); and severe hepatic impairment. It is contraindicated during the third trimester of pregnancy and in patients with known hypersensitivity to meloxicam or other NSAIDs, or those who experience bronchospasm, urticaria, or rhinitis following NSAID or aspirin exposure.

DRUG INTERACTIONS

Concurrent use of aspirin or other NSAIDs substantially increases the risk of gastrointestinal ulceration and bleeding and is not recommended. Anticoagulants, including warfarin and heparin, have an increased bleeding risk when combined with meloxicam. Antihypertensive drugs, including ACE inhibitors, ARBs, and diuretics, may have their efficacy attenuated. Lithium levels may be increased by meloxicam through reduced renal clearance, requiring monitoring. Methotrexate toxicity is increased as NSAIDs reduce methotrexate renal tubular secretion, particularly at high methotrexate doses.
