INDICATION

Xiton 1mg Tablet is an atypical antipsychotic indicated for the treatment of schizophrenia in adults and adolescents aged 13 years and older, covering both the acute management of psychotic episodes and long-term maintenance therapy to prevent relapse. It is also indicated for the short-term treatment of acute manic or mixed episodes associated with bipolar I disorder, as monotherapy or as an adjunct to mood stabilizers such as lithium or valproate. Additionally, Xiton 1mg is used for the management of irritability, aggression, self-injurious behavior, and severe temper tantrums in children and adolescents aged 5 to 16 years with autistic spectrum disorder. MECHANISM OF ACTION

Risperidone is an atypical antipsychotic that blocks dopamine D2 and serotonin 5-HT2A receptors. D2 antagonism in the mesolimbic pathway reduces positive symptoms of schizophrenia, such as hallucinations and delusions. 5-HT2A blockade in the mesocortical pathway enhances prefrontal dopamine, improving negative symptoms such as social withdrawal and cognitive deficits. It also interacts with alpha-1 adrenergic and histamine H1 receptors, causing orthostatic hypotension and sedation. This serotonin-dopamine balance provides broad antipsychotic efficacy. DOSAGE AND ADMINISTRATION

•      Schizophrenia in adults: Start at 1mg twice daily on day one, increase to 2mg twice daily on day two, then 3mg twice daily on day three as tolerated.

•      Schizophrenia in adolescents (13 to 17 years): Start at 0.5mg once daily, adjust by 0.5 to 1mg increments at intervals of at least 24 hours.

SIDE EFFECTS

•      Common: Weight gain, somnolence, headache, increased appetite, fatigue, constipation, dizziness.

•      Less common: Hyperprolactinemia causing menstrual irregularities, galactorrhea, gynecomastia; extrapyramidal symptoms including tremor, rigidity, and akathisia, especially at higher doses; elevated blood glucose.

DRUG INTERACTIONS

Carbamazepine and other CYP3A4 inducers can lower risperidone levels, requiring dose adjustments, while CYP2D6 inhibitors like fluoxetine and paroxetine can raise levels, necessitating dose reduction. Concurrent use with CNS depressants, such as alcohol or benzodiazepines, increases sedation and respiratory depression risk. QT-prolonging drugs can add to cardiac risks, and antihypertensives may enhance blood pressure lowering.
