INDICATION

KAIRDON 1MG TAB is an atypical antipsychotic indicated for the treatment of schizophrenia, including first episodes and relapse prevention. It is also used in the management of moderate to severe manic episodes associated with bipolar disorder, and for short-term treatment of persistent aggression in patients with Alzheimer's dementia or conduct disorder in children.

MECHANISM OF ACTION

Risperidone is a benzisoxazole derivative that acts as an antagonist at dopamine D2 receptors and serotonin 5-HT2A receptors. Blockade of D2 receptors in the mesolimbic pathway reduces positive symptoms of psychosis such as hallucinations and delusions. Simultaneous 5-HT2A antagonism in the prefrontal cortex improves negative symptoms including social withdrawal and emotional blunting, and reduces the extrapyramidal side effects associated with pure dopamine blockade. Risperidone also has activity at alpha-1 adrenergic and histamine H1 receptors.

DOSAGE AND ADMINISTRATION

•      Adults: 1mg twice daily on day 1; increase to 2mg twice daily on day 2 if tolerated

•      Maintenance: 2mg to 8mg per day in one or two divided doses

•      Elderly: start at 0.5mg twice daily and titrate slowly

•      Always follow the titration schedule prescribed by your psychiatrist; never stop abruptly

SIDE EFFECTS

Common:

•      Extrapyramidal symptoms: tremor, rigidity, akathisia (dose-dependent)

•      Somnolence and sedation

•      Weight gain and increased appetite

•      Hyperprolactinemia: galactorrhea, amenorrhea, sexual dysfunction

Less common:

•      Tardive dyskinesia with prolonged use

•      Orthostatic hypotension and dizziness, especially on initiation

•      QT prolongation (rare at therapeutic doses)

•      Metabolic effects: dyslipidemia, hyperglycemia, increased diabetes risk

NOT RECOMMENDED IN

•      Known hypersensitivity to risperidone or paliperidone

•      Elderly patients with dementia-related psychosis (increased risk of stroke and death)

•      Concurrent use with other antipsychotics without specialist guidance

•      Parkinson's disease or Lewy body dementia (severe sensitivity to antipsychotics)

DRUG INTERACTIONS

CNS depressants including alcohol, benzodiazepines, and opioids have additive sedative effects with risperidone; use with caution. CYP2D6 inhibitors such as fluoxetine and paroxetine increase risperidone plasma levels significantly; dose reduction may be required. Carbamazepine and other CYP3A4 inducers reduce risperidone levels, potentially causing loss of efficacy; monitor clinical response and adjust dose. Antihypertensives may have additive hypotensive effects due to risperidone's alpha-1 blocking activity.
