INDICATION

Xifaxa 550mg Tab is a minimally absorbed, gut-selective antibiotic indicated for the reduction in risk of overt hepatic encephalopathy (HE) recurrence in adults with liver cirrhosis, and for the treatment of irritable bowel syndrome with diarrhea (IBS-D) in adult patients. It is also used for traveler's diarrhea caused by non-invasive strains of Escherichia coli in adults and adolescents aged 12 years and older.

MECHANISM OF ACTION

Rifaximin is a gut-selective antibiotic that works by irreversibly inhibiting bacterial RNA synthesis, preventing bacteria from producing essential proteins needed for growth and survival. Due to its minimal systemic absorption, it remains concentrated within the gastrointestinal tract, where it specifically targets and reduces harmful intestinal bacteria. In hepatic encephalopathy, rifaximin lowers the population of ammonia-producing gut microbes, thereby reducing blood ammonia levels and helping prevent neuropsychiatric symptoms. In IBS-D, it helps restore microbial balance, decreasing bacterial overgrowth and fermentation, which leads to improvement in bloating, abdominal discomfort, and diarrhea.

DOSAGE AND ADMINISTRATION

•      Hepatic encephalopathy prevention: 550mg twice daily, taken long-term and continuously as directed by the treating hepatologist.

•      IBS-D treatment: 550mg three times daily for 14 days; treatment courses may be repeated if symptoms recur.

•      Traveler's diarrhea: 200mg three times daily for 3 days (lower strength tablets typically used for this indication).

SIDE EFFECTS

•      Common: Nausea, abdominal pain, flatulence, headache, peripheral edema in patients with underlying liver disease, and dizziness.

•      Less common: Fatigue, ascites worsening in cirrhotic patients, muscle spasms, pruritus, constipation.

NOT RECOMMENDED IN

Xifaxa 550mg Tab is contraindicated in patients with known hypersensitivity to rifaximin, other rifamycin-class antibiotics such as rifampicin, or any component of the formulation. It must not be used for diarrhea complicated by fever or blood in stool, or diarrhea caused by pathogens other than non-invasive E. coli, as its gut-restricted distribution makes it ineffective against systemic or invasive infections. Use in patients with severe hepatic impairment (Child-Pugh Class C) requires caution due to the potential for increased systemic absorption. Safety in pregnancy and breastfeeding has not been sufficiently established.

DRUG INTERACTIONS

Although minimally absorbed, rifaximin is a weak inducer of CYP3A4 and P-glycoprotein (P-gp). In patients with hepatic impairment where systemic absorption may be higher, rifaximin may reduce plasma levels of P-gp substrates, including digoxin and CYP3A4 substrates, including warfarin, cyclosporine, and tacrolimus; therapeutic monitoring is warranted. Ciclosporin has been reported to increase rifaximin systemic exposure through P-gp inhibition in liver disease patients. Antacids are unlikely to produce clinically significant interactions.
