INDICATION

Xenglu Met 12.5mg+500mg Tab is a fixed-dose combination SGLT2 inhibitor and biguanide indicated for the management of type 2 diabetes mellitus in adults. It provides glycaemic control with the additional cardiovascular and renal protective benefits of empagliflozin.

MECHANISM OF ACTION

Empagliflozin selectively inhibits sodium-glucose cotransporter 2 (SGLT2) in the proximal tubule of the kidney, blocking reabsorption of filtered glucose and promoting urinary glucose excretion, thereby reducing blood glucose independently of insulin. This insulin-independent mechanism reduces the risk of Hypoglycaemia and promotes modest weight loss and blood pressure reduction. Metformin reduces hepatic glucose output and improves peripheral insulin sensitivity. The combination provides complementary glucose-lowering with cardiovascular and renal organ protection from the empagliflozin component.

DOSAGE AND ADMINISTRATION

•      Adults: 1 tablet twice daily with meals

•      Renal function (eGFR) must be checked before initiation and regularly thereafter

•      Swallow whole with water

•      Always follow the dosage prescribed by your doctor

SIDE EFFECTS

Common:

•      Genital mycotic infections (thrush) in both sexes (empagliflozin-related)

•      Urinary tract infections (empagliflozin-related)

•      Nausea and diarrhoea (metformin-related)

•      Polyuria (increased urination)

Less common:

•      Diabetic ketoacidosis (rare but serious; euglycemic DKA possible with SGLT2 inhibitors)

DRUG INTERACTIONS

Diuretics combined with SGLT2 inhibitors increase volume depletion risk and hypotension; monitor closely. Iodinated contrast: stop metformin 48 hours before. Insulin and sulfonylureas: Hypoglycaemia risk increases; dose reduction may be needed. Alcohol: lactic acidosis risk with metformin.
