INDICATION

Xavor Diu 50/12.5mg Tab is a fixed-dose combination antihypertensive indicated for the management of hypertension in adults whose blood pressure is not adequately controlled on losartan monotherapy alone, or as initial therapy in patients who are likely to require more than one antihypertensive agent to reach blood pressure targets. The combination of losartan and hydrochlorothiazide provides complementary and additive blood pressure reduction through two mechanistically distinct pathways, offering more effective and sustained control than either agent used alone. It is also indicated in hypertensive patients with left ventricular hypertrophy to reduce the risk of stroke, where losartan has demonstrated superior cardiovascular outcome benefits over beta-blocker-based therapy in landmark clinical trials such as LIFE.

MECHANISM OF ACTION

Xavor Diu works through two synergistic mechanisms to lower blood pressure. Losartan is a selective angiotensin II type 1 (AT1) receptor antagonist that prevents angiotensin II, the most potent endogenous vasoconstrictor, from binding to its receptor on vascular smooth muscle cells and the adrenal cortex. Blocking AT1 receptors prevents vasoconstriction, reduces aldosterone secretion (which otherwise promotes sodium and water retention in the kidney's collecting duct), and attenuates sympathetic nervous system activation triggered by angiotensin II, resulting in vasodilation and mild natriuresis. Hydrochlorothiazide is a thiazide diuretic that inhibits the sodium-chloride cotransporter (NCC) in the distal convoluted tubule of the nephron, reducing sodium and chloride reabsorption and increasing urinary excretion of fluid, thereby lowering blood volume and cardiac preload.

DOSAGE AND ADMINISTRATION

SIDE EFFECTS

It is contraindicated in patients with anuria, severe renal impairment (eGFR below 30ml/min), severe hepatic impairment, and in patients with known hypersensitivity to sulfonamide-derived compounds, which includes hydrochlorothiazide. Concurrent use with aliskiren is contraindicated in patients with diabetes mellitus or renal impairment. A prior history of angioedema with any ARB is a contraindication to losartan use.

DRUG INTERACTIONS

Potassium-sparing diuretics, potassium supplements, and potassium-containing salt substitutes combined with losartan may cause dangerous hyperkalemia; close monitoring is required, especially in patients with renal impairment. NSAIDs blunt the antihypertensive and diuretic effects and increase the risk of acute kidney injury, particularly in elderly patients and those with compromised renal function. Thiazide diuretics reduce lithium renal clearance, increasing the risk of lithium toxicity; close monitoring and possible dose reduction of lithium are needed. Insulin and oral antidiabetic drugs may require dose adjustment as hydrochlorothiazide can impair glucose tolerance. Concomitant antihypertensives produce additive blood pressure-lowering effects.
