INDICATION

Wilgesic Tab is a combination analgesic and centrally acting muscle relaxant indicated for the relief of mild to moderate musculoskeletal pain including back pain, neck pain, muscle strain, and conditions where skeletal muscle spasm contributes to pain and stiffness.

MECHANISM OF ACTION

Paracetamol at 450mg provides central analgesic activity by inhibiting prostaglandin synthesis in the CNS without significant peripheral anti-inflammatory activity. Orphenadrine citrate at 35mg is a centrally acting anticholinergic that reduces skeletal muscle tone and spasm by inhibiting acetylcholine at central cholinergic receptors, without direct peripheral neuromuscular blockade. The combination provides dual-mechanism pain relief addressing both the pain signal and the muscular spasm component of musculoskeletal conditions.

DOSAGE AND ADMINISTRATION

•      Adults: 1 to 2 tablets two to three times daily as needed

•      Do not combine with other paracetamol-containing products

•      May be taken with food to reduce stomach irritation

•      Always follow the dosage prescribed by your doctor; avoid in hepatic impairment and with alcohol

SIDE EFFECTS

Common:

•      Dry mouth (orphenadrine anticholinergic effect)

•      Mild drowsiness

•      Constipation

•      Headache

Less common:

•      Urinary retention in men with prostatic hypertrophy

•      Blurred vision

•      Mild tachycardia

•      Confusion in elderly patients

DRUG INTERACTIONS

Anticholinergic drugs produce additive side effects with orphenadrine. Paracetamol combined with alcohol or hepatotoxic drugs increases liver damage risk. CNS depressants add to orphenadrine sedation. Warfarin: monitor INR with prolonged regular paracetamol use.
