INDICATION

Vrontil 10mg Tab is a multimodal antidepressant indicated for the treatment of major depressive disorder (MDD) in adults. It is particularly useful in patients with cognitive dysfunction associated with depression, including difficulties with memory, concentration, and processing speed.

MECHANISM OF ACTION

Vortioxetine has a unique multimodal mechanism combining serotonin transporter (SERT) inhibition with direct activity at multiple serotonin receptor subtypes. This combination of SERT inhibition and receptor modulation enhances serotonergic neurotransmission while also increasingly dopaminergic, noradrenergic, histaminergic, ll acetyl cholinergic activity through indirect mechanisms. The modulation of multiple neurotransmitter systems is thought to contribute to its procognitive effects, which are more pronounced than with conventional SSRIs.

DOSAGE AND ADMINISTRATION

•      Initial dose: 10mg once daily, with or without food

•      May be increased to 20mg once daily based on clinical response after at least 2 weeks

•      May be reduced to 5mg if 10mg is not tolerated

•      Always follow the dosage and titration schedule prescribed by your psychiatrist

SIDE EFFECTS

Common:

•      Nausea (most common, particularly in the first 1 to 2 weeks)

•      Headache and dizziness

•      Diarrhoea and dry mouth

•      Pruritus (itching)

Less common:

•      Sexual dysfunction

•      Abnormal dreams and insomnia

•      Hyponatremia, particularly in elderly patients

•      Serotonin syndrome when combined with other serotonergic drugs (rare)

NOT RECOMMENDED IN

•      Concurrent use with MAO inhibitors (14-day washout required before and after vortioxetine)

•      Known hypersensitivity to vortioxetine

•      Children and adolescents under 18 years (safety not established)

•      Caution in patients with a history of seizures or mania

DRUG INTERACTIONS

MAO inhibitors are contraindicated. Rifampicin (CYP inducer) may halve vortioxetine plasma levels; consider dose increase to maximum 20mg if combined long-term. Strong CYP2D6 inhibitors such as bupropion and fluoxetine increase vortioxetine levels significantly; reduce dose to 5mg if combined. Concurrent serotonergic drugs increase serotonin syndrome risk. NSAIDs and anticoagulants increase bleeding risk.
