INDICATION

Vrontil 5mg Tab is a multimodal antidepressant used as the starting dose for treatment-naive patients beginning vortioxetine, and as a maintenance dose in patients taking strong CYP2D6 inhibitors who require dose reduction. It is indicated for major depressive disorder in adults.

MECHANISM OF ACTION

At 5mg, vortioxetine provides initial serotonin transporter inhibition and partial engagement of its receptor targets, establishing tolerable antidepressant activity while allowing patients to adjust to the medication. This dose achieves approximately 50% SERT occupancy. The 5mg dose is the dose reduction target recommended in patients on strong CYP2D6 inhibitors such as bupropion or fluoxetine, which significantly increase vortioxetine plasma levels.

DOSAGE AND ADMINISTRATION

•      Adults: 5mg once daily as a starting dose, particularly in elderly or sensitive patients

•      Titrate upward to 10mg after at least 1 week if tolerated

•      Use as maintenance dose when prescribed with strong CYP2D6 inhibitors

•      Always follow the dosage prescribed by your psychiatrist

SIDE EFFECTS

Common:

•      Nausea (mild at 5mg; usually well tolerated)

•      Headache

•      Dry mouth

•      Mild dizziness

Less common:

•      Sexual dysfunction

•      Sleep disturbances

•      Hyponatremia in elderly patients

•      Pruritus

NOT RECOMMENDED IN

•      Concurrent MAO inhibitors

•      Known hypersensitivity to vortioxetine

•      Children and adolescents under 18 years

•      Not a therapeutic dose for most patients without CYP2D6 inhibitor use; titrate as tolerated

DRUG INTERACTIONS

MAO inhibitors contraindicated. Strong CYP2D6 inhibitors (bupropion, fluoxetine, paroxetine) markedly increase vortioxetine exposure; 5mg is the appropriate dose when these are co-prescribed. Rifampicin reduces vortioxetine levels. Serotonergic drugs increase serotonin syndrome risk at all doses.
