INDICATION

Vrontil 20mg Tab is the maximum recommended dose of vortioxetine, a multimodal antidepressant indicated for major depressive disorder in adults. It is used in patients with an incomplete response to 10mg and is associated with the greatest procognitive benefits.

MECHANISM OF ACTION

At 20mg, vortioxetine achieves maximal SERT inhibition alongside the most complete engagement of its target serotonin receptor subtypes (5-HT3 antagonism, 5-HT1A agonism, 5-HT7 antagonism). The downstream enhancement of multiple neurotransmitter systems is at its peak, producing the most pronounced antidepressant and procognitive effects. Clinical trials have demonstrated significantly better cognitive performance at 20mg versus placebo, making this dose particularly appropriate for patients where cognitive symptoms are a prominent feature of their depression.

DOSAGE AND ADMINISTRATION

•      Adults: 20mg once daily, with or without food

SIDE EFFECTS

Common:

•      Nausea (most likely at 20mg, particularly in the first 1 to 2 weeks)

DRUG INTERACTIONS

MAO inhibitors contraindicated. Strong CYP2D6 inhibitors increase vortioxetine levels markedly at 20mg; reduce to 5mg if co-prescribed. Rifampicin reduces levels; may need to maintain at 20mg rather than reducing. Concurrent serotonergic agents increase serotonin syndrome risk. NSAIDs and anticoagulants increase bleeding risk.
