INDICATION

Vepridone 2mg Tab is an atypical antipsychotic indicated for the treatment of schizophrenia, acute and maintenance, and for manic episodes in bipolar disorder. The 2mg dose is used during titration to therapeutic doses in new patients and as maintenance in patients with lower dose requirements.

MECHANISM OF ACTION

Risperidone is a combined dopamine D2 and serotonin 5-HT2A receptor antagonist. D2 blockade in mesolimbic pathways reduces positive psychotic symptoms, while 5-HT2A antagonism in the prefrontal cortex improves negative symptoms and reduces extrapyramidal side effects compared to typical antipsychotics. The active metabolite 9-hydroxyrisperidone (paliperidone) contributes substantially to the overall antipsychotic effect. At 2mg, risperidone achieves D2 receptor occupancy in the lower therapeutic range, providing meaningful antipsychotic activity with a relatively lower risk of extrapyramidal effects.

DOSAGE AND ADMINISTRATION

•      Adults: titrate from 1mg on day 1 to 2mg on day 2 as tolerated

•      Maintenance: 2mg daily may be sufficient for some patients; typical range 4 to 6mg per day

•      Take with or without food

•      Always follow the titration and maintenance schedule prescribed by your psychiatrist

SIDE EFFECTS

Common:

•      Extrapyramidal symptoms at therapeutic doses

•      Somnolence and fatigue

•      Weight gain

•      Hyperprolactinemia

Less common:

•      Orthostatic hypotension

•      Tardive dyskinesia with prolonged use

•      QT prolongation

•      Metabolic effects with long-term use

•      Elderly patients with dementia-related psychosis

DRUG INTERACTIONS

CYP2D6 inhibitors (fluoxetine, paroxetine) increase risperidone plasma levels significantly; monitor for side effects and consider dose reduction. Carbamazepine reduces risperidone levels through CYP3A4 induction. CNS depressants add to sedation. Antihypertensives produce additive hypotension.
