INDICATION

Trioptal 600mg Tab is an antiepileptic medication indicated for the treatment of partial-onset seizures (also called focal seizures) with or without secondary generalization in adults and children aged six years and above. It is used both as monotherapy in newly diagnosed epilepsy and as adjunctive therapy when additional seizure control is required in patients not fully controlled on existing antiepileptic regimens. It may also be used in the management of trigeminal neuralgia in some cases. Trioptal 600mg significantly reduces the frequency and severity of seizures, helping patients achieve better epilepsy control and improved quality of life.

MECHANISM OF ACTION

Oxcarbazepine works primarily by blocking voltage-gated sodium channels in brain neurons. These channels are responsible for generating the electrical signals (action potentials) that neurons use to communicate. In epilepsy, neurons fire excessively and repetitively, causing seizures. By stabilizing sodium channels in their inactive state, oxcarbazepine prevents rapid, high-frequency neuronal firing and reduces abnormal electrical activity in the brain. It also inhibits the release of glutamate, an excitatory neurotransmitter that promotes seizure propagation. Oxcarbazepine is a prodrug that is converted in the body into its active form, the monohydroxy derivative (MHD), which produces these effects.

DOSAGE AND ADMINISTRATION

•      Adults (monotherapy): Starting dose 300mg twice daily, increasing by up to 600mg per week to a usual maintenance of 1200mg per day.

•      Adults (adjunctive therapy): Starting dose 300mg twice daily, with gradual increases to up to 2400mg per day as tolerated.

•      Do not stop abruptly; always taper under physician supervision to avoid seizure rebound.

SIDE EFFECTS

•      Common: Dizziness, somnolence, headache, nausea, diplopia (double vision), fatigue.

•      Less common: Hyponatremia (low sodium), ataxia, tremor, vomiting, skin rash, Stevens-Johnson syndrome (rare but serious).

NOT RECOMMENDED IN

Trioptal 600mg Tab is not recommended in patients with known hypersensitivity to oxcarbazepine or carbamazepine, as significant cross-reactivity exists. Hyponatremia is a serious risk; sodium levels should be monitored, particularly in elderly patients and those on diuretics or other sodium-depleting medications. Not suitable for patients with severe hepatic impairment. Use during pregnancy requires specialist evaluation, as oxcarbazepine may increase the risk of fetal malformations; effective contraception is essential for women of childbearing potential.

DRUG INTERACTIONS

Oxcarbazepine is a significant inducer of CYP3A4 and reduces the plasma levels of oral contraceptives, making them less effective; alternative contraception is essential. It lowers the efficacy of other CYP3A4-metabolized drugs, including some antiepileptics, immunosuppressants, and calcium channel blockers. CYP2C19 inhibitors, including felodipine and verapami,l may increase oxcarbazepine levels. Combination with diuretics, NSAIDs, or other medications that lower sodium increases the risk of hyponatremia.
