INDICATION

Tagipmet XR 50/500mg Tab is an extended-release fixed-dose combination indicated for type 2 diabetes mellitus in adults who require the lower metformin dose (500mg) due to gastrointestinal sensitivity or as an initiation dose when adding sitagliptin to metformin therapy at a lower metformin level.

MECHANISM OF ACTION

Sitagliptin 50mg inhibits DPP-4 and enhances incretin-mediated glycaemic control. Extended-release metformin 500mg provides hepatic glucose production inhibition and peripheral insulin sensitization at a lower dose, suitable for initiation or for patients with gastrointestinal sensitivity to higher metformin doses. The extended-release delivery of this lower metformin dose minimizes gastrointestinal side effects while establishing the DPP-4 inhibitor component of the combination.

DOSAGE AND ADMINISTRATION

•      Adults: 1 tablet once daily with the evening meal initially

•      Metformin dose may be uptitrated to higher strength combinations as tolerated

•      Swallow whole; do not crush or split

•      Always follow the dosage and titration schedule prescribed by your doctor

SIDE EFFECTS

Common:

•      Nausea (reduced with XR formulation at 500mg dose)

•      Nasopharyngitis

•      Headache

•      Mild abdominal discomfort

Less common:

•      Lactic acidosis (rare; lower risk at 500mg metformin dose)

•      Pancreatitis (rare)

•      Vitamin B12 deficiency with long-term use

•      Hypoglycaemia with concurrent sulfonylurea or insulin

DRUG INTERACTIONS

Iodinated contrast: stop metformin before procedure. Alcohol: lactic acidosis risk. Insulin and sulfonylureas: hypoglycemia risk. Cimetidine reduces metformin renal clearance. ACE inhibitors may enhance sitagliptin efficacy.
