INDICATION

Solutina 100mg Inj is a corticosteroid indicated for emergency and short-term parenteral management of severe inflammatory, allergic, and immunological conditions. It is used in acute adrenocortical insufficiency (Addisonian crisis), anaphylaxis, severe asthma, acute exacerbations of inflammatory bowel disease, and other conditions requiring rapid systemic glucocorticoid therapy when oral administration is not possible.

MECHANISM OF ACTION

Hydrocortisone is the synthetic form of the endogenous adrenal cortisol hormone. It binds to intracellular glucocorticoid receptors, forming a complex that translocates to the nucleus and modulates transcription of anti-inflammatory and immunosuppressive genes. It suppresses pro-inflammatory cytokine production, reduces vascular permeability, inhibits leukocyte migration to sites of inflammation, and stabilises mast cells and lysosomal membranes. The sodium succinate ester is water-soluble, enabling rapid intravenous or intramuscular administration for immediate systemic effect in acute and emergency settings.

DOSAGE AND ADMINISTRATION

•      Acute adrenal insufficiency: 100mg by slow IV injection immediately, then 100mg every 6 to 8 hours

•      Anaphylaxis and severe asthma: 100 to 500mg by slow IV injection depending on severity

•      Administer by slow intravenous injection or infusion; intramuscular injection may also be used

•      Always administered by a trained healthcare professional; dose and frequency determined by the prescribing physician

SIDE EFFECTS

Common:

•      Elevated blood glucose, particularly in diabetic patients

•      Fluid retention and sodium retention causing oedema

•      Elevated blood pressure

•      Flushing and insomnia with repeat dosing

Less common:

•      Adrenal suppression with repeated or prolonged use

•      Increased susceptibility to infections

•      Peptic ulceration, particularly with concurrent NSAID use

DRUG INTERACTIONS

NSAIDs combined with hydrocortisone significantly increase the risk of gastrointestinal ulceration and bleeding. CYP3A4 inhibitors such as ketoconazole and clarithromycin increase hydrocortisone plasma levels, enhancing both therapeutic and adverse effects. CYP3A4 inducers including rifampicin, carbamazepine, and phenytoin reduce hydrocortisone efficacy. Antidiabetic drug doses may require adjustment as corticosteroids raise blood glucose. Potassium-depleting diuretics combined with hydrocortisone increase hypokalaemia risk.
