INDICATION

Softin-P Tab is a combination antihistamine and decongestant indicated for the symptomatic relief of allergic rhinitis with nasal congestion. It provides relief from sneezing, runny nose, itchy eyes, and nasal stuffiness in conditions such as seasonal and perennial allergic rhinitis and sinusitis with congestion.

MECHANISM OF ACTION

Loratadine is a second-generation non-sedating H1 histamine receptor antagonist that blocks peripheral histamine receptors, reducing allergy symptoms such as sneezing, itching, and rhinorrhea without significant CNS penetration. Pseudoephedrine is a sympathomimetic amine that acts on alpha-adrenergic receptors in the nasal mucosa, causing vasoconstriction and reducing mucosal swelling and nasal congestion. The combination addresses both the histamine-mediated inflammatory component and the vascular congestion component of allergic rhinitis.

DOSAGE AND ADMINISTRATION

•      Adults and children over 12 years: 1 tablet twice daily (every 12 hours)

•      Take with or without food

•      Do not crush or chew sustained-release formulations

•      Always follow the dosage prescribed by your doctor; avoid use in the evening if insomnia occurs

SIDE EFFECTS

Common:

•      Insomnia and restlessness (pseudoephedrine-related)

•      Headache

•      Dry mouth

•      Nausea or stomach discomfort

Less common:

•      Elevated blood pressure and heart rate (pseudoephedrine)

•      Palpitations and tachycardia

•      Urinary retention in men with prostatic hypertrophy

•      Anxiety and irritability

NOT RECOMMENDED IN

•      Hypertension (pseudoephedrine can raise blood pressure)

•      Cardiovascular disease, hyperthyroidism, or diabetes

•      Concurrent MAO inhibitors or within 14 days of stopping them (hypertensive crisis risk)

•      Severe renal impairment

DRUG INTERACTIONS

MAO inhibitors combined with pseudoephedrine can cause hypertensive crisis; contraindicated within 14 days. Beta-blockers antagonize pseudoephedrine's cardiovascular effects and can cause severe hypertension; avoid. Tricyclic antidepressants and methylphenidate enhance pseudoephedrine's sympathomimetic effects. Loratadine levels may be increased by CYP3A4 inhibitors, but this is rarely clinically significant. Avoid concurrent use with other decongestants.
