INDICATION

Phylin 400mg Tab is a xanthine bronchodilator indicated for the long-term treatment of reversible airways obstruction in bronchial asthma and chronic obstructive pulmonary disease (COPD). It is used as an adjunct to inhaled therapy when symptoms are not adequately controlled by beta-2 agonists and inhaled corticosteroids alone.

MECHANISM OF ACTION

Doxofylline is a newer xanthine derivative that produces bronchodilation by inhibiting phosphodiesterase enzymes, particularly PDE4, leading to increased intracellular cyclic AMP in bronchial smooth muscle cells. Elevated cAMP activates protein kinase A, which phosphorylates and inhibits myosin light chain kinase, causing smooth muscle relaxation and bronchodilation. Unlike theophylline, Doxofylline has a lower affinity for adenosine receptors, which significantly reduces its cardiovascular side effects (tachycardia, arrhythmias) and CNS stimulation (insomnia, seizures), while maintaining similar bronchodilatory efficacy.

DOSAGE AND ADMINISTRATION

•      Adults: 400mg two to three times daily

•      Take after meals to reduce gastrointestinal side effects

•      Avoid foods and beverages that affect xanthine metabolism (excessive caffeine)

•      Always follow the dosage prescribed by your pulmonologist; therapeutic drug monitoring is not typically required for Doxofylline

SIDE EFFECTS

Common:

•      Nausea, vomiting, stomach upset

•      Headache

•      Irritability and restlessness (less than theophylline)

•      Palpitations (less frequent than theophylline)

Less common:

•      Tachycardia and arrhythmias at excessive doses

DRUG INTERACTIONS

Cimetidine, erythromycin, and fluoroquinolones inhibit xanthine metabolism, increasing Doxofylline plasma levels and risk of toxicity. Rifampicin and carbamazepine induce hepatic metabolism, reducing Doxofylline efficacy. Caffeine has additive CNS and cardiovascular stimulant effects; advise patients to limit coffee and tea. Beta-blockers may antagonize the bronchodilatory effect in asthma patients.
