INDICATION

P-Nec 12.5 XR Tab is an extended-release SSRI indicated for the treatment of major depressive disorder, panic disorder with or without agoraphobia, social anxiety disorder, generalized anxiety disorder, and post-traumatic stress disorder (PTSD). The 12.5mg XR dose is used as an initial starting dose in patients who are sensitive to SSRIs or require careful titration.

MECHANISM OF ACTION

Paroxetine selectively inhibits the reuptake of serotonin into presynaptic neurons by blocking the serotonin transporter (SERT), increasing serotonin availability in the synaptic cleft. It also has mild muscarinic acetylcholine receptor antagonism and weak norepinephrine reuptake inhibition, contributing to its efficacy across anxiety and depressive disorders. The extended-release formulation uses a controlled-release polymer matrix that delivers paroxetine gradually over 24 hours, reducing peak plasma concentration-related side effects such as nausea and improving tolerability, particularly at treatment initiation.

DOSAGE AND ADMINISTRATION

•      Initial dose: 12.5mg once daily in the morning, with or without food

•      Dose may be increased by 12.5mg increments at weekly intervals based on clinical response

•      Usual therapeutic range: 12.5mg to 62.5mg once daily depending on indication

•      Always follow the titration schedule prescribed by your psychiatrist; never stop abruptly as withdrawal syndrome can occur

SIDE EFFECTS

Common:

DRUG INTERACTIONS

MAO inhibitors are strictly contraindicated; a 14-day washout is required in both directions. Paroxetine is a potent CYP2D6 inhibitor and significantly increases plasma levels of tricyclic antidepressants, antipsychotics including risperidone, propafenone, flecainide, and tamoxifen (reducing tamoxifen efficacy in breast cancer treatment). Concurrent serotonergic agents including tramadol, triptans, linezolid, and lithium increase serotonin syndrome risk. Anticoagulant effect of warfarin may be enhanced; monitor INR closely.
