INDICATION

Novapressin 1mg Inj is a synthetic vasopressin analogue indicated for the treatment of bleeding esophageal varices in patients with portal hypertension due to liver cirrhosis. It is also used in hepatorenal syndrome to improve renal function and in the management of septic shock when norepinephrine is insufficient.

MECHANISM OF ACTION

Terlipressin is a prodrug that is enzymatically converted to the active compound lysine-vasopressin in body tissues. Lysine-vasopressin acts on vasopressin V1 receptors on vascular smooth muscle, causing splanchnic and systemic vasoconstriction. In variceal bleeding, this reduces portal blood flow and portal pressure, decreasing hemorrhage from esophageal varices. In hepatorenal syndrome, it improves effective arterial blood volume and renal perfusion by reversing splanchnic vasodilation. Terlipressin has a longer half-life than vasopressin, allowing intermittent bolus dosing.

DOSAGE AND ADMINISTRATION

•      Variceal bleeding: 1 to 2mg by slow intravenous injection every 4 to 6 hours until hemostasis, up to 72 hours

•      Hepatorenal syndrome: 0.5 to 1mg every 4 to 6 hours, titrated based on response

•      Administered only by intravenous injection by trained healthcare professionals

•      Always prescribed and monitored by a specialist; not for self-administration

SIDE EFFECTS

Common:

•      Abdominal cramps and diarrhoea (vasoconstriction of intestinal vessels)

•      Hypertension (elevated blood pressure)

•      Pallor and sweating

•      Headache

Less common:

•      Cardiac arrhythmias and bradycardia

•      Peripheral and mesenteric ischemia

•      Respiratory failure (serious; monitor oxygen saturation continuously)

•      Hyponatremia with prolonged use

DRUG INTERACTIONS

Terlipressin combined with drugs that cause bradycardia, such as beta-blockers and digoxin, increases the risk of serious cardiac arrhythmias; monitor ECG during co-administration. NSAIDs may reduce the renal benefits of Terlipressin in hepatorenal syndrome. Concurrent use with other vasopressors such as norepinephrine requires careful hemodynamic monitoring to avoid excessive hypertension. Albumin infusion is commonly co-administered with Terlipressin in hepatorenal syndrome to improve efficacy.
