INDICATION

Nitrofurantoin 100mg is a urinary tract antibiotic indicated for the treatment and prophylaxis of uncomplicated urinary tract infections (UTIs) caused by susceptible strains of Escherichia coli, Enterococcus, Staphylococcus saprophyticus, and other gram-negative uropathogens. It is particularly effective for lower UTIs, including cystitis in adult women, where its high urinary concentrations deliver targeted antibacterial action. Nitrofurantoin is also used as long-term suppressive therapy in patients with recurrent UTIs or structural abnormalities predisposing to infection.

MECHANISM OF ACTION

Nitrofurantoin is reduced by bacterial nitroreductase enzymes to highly reactive intermediates that cause widespread damage to multiple bacterial targets. These reactive species attack bacterial DNA, RNA, ribosomal proteins, and cell wall synthesis pathways simultaneously, making resistance development through single-gene mutations highly unlikely. This multi-target mechanism distinguishes nitrofurantoin from single-mechanism antibiotics. The drug achieves very high urinary concentrations (50–200 times plasma concentrations) while remaining below systemic therapeutic levels, providing organ-specific antibacterial activity confined to the urinary tract.

DOSAGE AND ADMINISTRATION

•      Acute UTI treatment: 50–100mg four times daily for 7 days (with food to improve tolerability).

•      Prophylaxis: 50–100mg once nightly for long-term suppression.

•      Always take with food or milk to reduce gastrointestinal side effects.

•      Drink plenty of fluids during treatment to promote urinary flushing.

•      Dose reduction or discontinuation required if creatinine clearance falls below 45 mL/min.

Quinolone antibiotics can interfere with their antibacterial action in the urinary tract and should not be combined.
