INDICATION

Morcet 20mg Tablets contain escitalopram, a selective serotonin reuptake inhibitor (SSRI) indicated for the treatment of major depressive disorder (MDD) in adults and adolescents aged 12 years and above, generalized anxiety disorder (GAD), social anxiety disorder, panic disorder with or without agoraphobia, and obsessive-compulsive disorder (OCD). Escitalopram is the therapeutically active S-enantiomer of citalopram and is one of the most widely prescribed antidepressants due to its favorable tolerability profile.

MECHANISM OF ACTION

Escitalopram selectively inhibits the reuptake of serotonin (5-HT) at the presynaptic neuronal membrane by binding to the serotonin transporter (SERT). This increases the concentration of serotonin in the synaptic cleft, enhancing serotonergic neurotransmission. Escitalopram has negligible affinity for norepinephrine and dopamine transporters, and minimal binding to histaminergic, muscarinic, and adrenergic receptors, which accounts for its cleaner side effect profile compared to tricyclic antidepressants. Clinical improvement typically begins within 1 to 2 weeks, with full therapeutic effect reached at 4 to 6 weeks.

DOSAGE AND ADMINISTRATION

·        Adults: start with 10mg once daily, increasing to 20mg daily after at least 1 week based on clinical response

·        Elderly patients and those with hepatic impairment: 10mg once daily is the recommended maximum dose

·        Adolescents (12 to 17 years): initial dose of 10mg once daily; increase to 20mg only after 3 weeks if needed

·        Take at the same time each day, with or without food

·        Do not discontinue abruptly; taper gradually over at least 1 to 2 weeks to prevent withdrawal symptoms

SIDE EFFECTS

Common:

·        Nausea, which is the most frequently reported side effect, usually diminishing within the first 2 weeks

·        Headache and dizziness

·        Insomnia or drowsiness

·        Dry mouth and increased sweating

Less common:

·        Sexual dysfunction including decreased libido, delayed ejaculation, and anorgasmia

·        Serotonin syndrome when combined with other serotonergic agents, presenting as agitation, hyperthermia, and tremor

·        Hyponatremia, particularly in elderly patients or those on diuretics

NOT RECOMMENDED IN

·        Known hypersensitivity to escitalopram or citalopram

·        Concurrent use with monoamine oxidase inhibitors (MAOIs), or within 14 days of stopping an MAOI

·        Concurrent use with pimozide due to QT prolongation risk

·        Patients with known QT prolongation or congenital long QT syndrome

DRUG INTERACTIONS

Concurrent use with MAOIs is contraindicated due to the risk of life-threatening serotonin syndrome. Serotonergic drugs including triptans, tramadol, tryptophan, and St. John's Wort increase serotonin syndrome risk. CYP2C19 inhibitors (omeprazole, fluconazole) may increase escitalopram levels. NSAIDs, aspirin, and anticoagulants increase bleeding risk. Escitalopram may increase levels of metoprolol and desipramine.
