INDICATION

Miro 5mg is indicated for the treatment of peripheral neuropathic pain in adults, including diabetic peripheral neuropathic pain (DPNP) and postherpetic neuralgia (PHN). It provides effective analgesia for patients suffering from chronic nerve pain resulting from diabetes-related nerve damage or following a herpes zoster infection. Miro also helps reduce the intensity and frequency of burning, shooting, or electric-shock sensations associated with neuropathy.

MECHANISM OF ACTION

Mirogabalin works by attaching to a specific part of nerve cells in the central nervous system called the alpha-2-delta-1 subunit of calcium channels. This reduces the flow of calcium into overactive pain-sensing nerves, which in turn lowers the release of chemicals that cause pain, such as glutamate, substance P, and norepinephrine. By calming these overactive nerves without affecting normal nerve signals, mirogabalin helps reduce chronic nerve pain. It also boosts the body’s natural pain-blocking pathways, giving extra relief from discomfort.

DOSAGE AND ADMINISTRATION

•      Starting dose: 5mg twice daily; titrated by 5mg increments at weekly intervals.

DRUG INTERACTIONS

Thiazolidinedione antidiabetics such as pioglitazone may worsen mirogabalin-induced peripheral edema and weight gain. Antiepileptics with similar mechanisms (e.g., gabapentin, pregabalin) should not be combined with mirogabalin due to additive adverse effects, and no demonstrated additional benefit.
