INDICATION
Gneb 2.5mg is indicated for the treatment of essential hypertension in adults and as adjunctive therapy in chronic stable heart failure in patients aged 70 years and above. It reduces blood pressure and cardiovascular risk through a unique vasodilating beta-blocker mechanism.
MECHANISM OF ACTION
Nebivolol is a third-generation, highly selective beta-1 adrenergic receptor antagonist with vasodilating properties mediated by nitric oxide (NO) release from the endothelium. Beta-1 blockade reduces heart rate, cardiac output, and myocardial oxygen demand. The NO-mediated vasodilation reduces peripheral vascular resistance, further lowering blood pressure. This dual mechanism distinguishes nebivolol from older beta-blockers and results in favorable hemodynamic and metabolic effects including preservation of glucose tolerance and lipid profiles.
DOSAGE AND ADMINISTRATION
● Adults: 2.5mg once daily, which may be increased to 5mg daily after 2 weeks based on response
● For heart failure: start at 1.25mg daily and titrate slowly under medical supervision
● Take at the same time each day, with or without food
● Swallow whole with water
● Do not discontinue abruptly; taper over 1 to 2 weeks
● Dose adjustment required in hepatic or renal impairment
SIDE EFFECTS
Common:
● Headache
● Fatigue or dizziness
● Bradycardia
Less common:
● Peripheral edema
● Nausea or diarrhea
● Cold extremities
● Worsening of heart failure symptoms at initiation
NOT RECOMMENDED IN
● Severe bradycardia or heart rate below 60 bpm
● Uncontrolled heart failure
● Cardiogenic shock
● Severe hepatic impairment
● Asthma or severe COPD
● Known hypersensitivity to nebivolol
DRUG INTERACTIONS
Calcium channel blockers such as verapamil may cause severe bradycardia or heart block. Clonidine withdrawal may be exaggerated when combined with beta-blockers. CYP2D6 inhibitors such as fluoxetine and paroxetine may increase nebivolol plasma levels. Concurrent antihypertensives may have additive blood pressure lowering effects. Insulin-treated diabetics should be aware that nebivolol may mask hypoglycemia symptoms.
