INDICATION

Frusemide Inj is a loop diuretic used in the emergency and hospital management of acute pulmonary oedema, acute heart failure, severe hypertension, acute renal failure, oedema associated with chronic renal failure or nephrotic syndrome, and hypertensive crisis. The injectable form provides rapid onset of diuresis (within 15 minutes of IV administration) in situations where oral therapy is insufficient or contraindicated.

MECHANISM OF ACTION

Furosemide inhibits the sodium-potassium-chloride (Na-K-2Cl) cotransporter in the thick ascending loop of Henle, blocking reabsorption of sodium, chloride, potassium, and water. This produces a powerful, dose-dependent diuresis and natriuresis, rapidly reducing plasma volume and oedema. It also has direct venodilatory effects, reducing preload before diuresis begins. Furosemide inhibits carbonic anhydrase and directly dilates renal blood vessels, increasing renal blood flow.

DOSAGE AND ADMINISTRATION

•      Acute pulmonary oedema: 40mg IV slowly over 1 to 2 minutes initially; may repeat with 80mg after 1 hour if needed

•      Oedema in renal/hepatic disease: 20mg to 40mg IV or IM; adjust based on response

•      Hypertensive crisis: 20mg to 40mg IV slow bolus

•      Do not mix with solutions of pH below 5.5 (precipitation occurs)

•      Monitor urine output, electrolytes, and renal function closely

SIDE EFFECTS

Common:

•      Hypokalaemia, hyponatremia, hypomagnesaemia

•      Hypotension with rapid IV injection

•      Dehydration and elevated serum creatinine

•      Muscle cramps

Less common:

•      Ototoxicity (tinnitus, hearing loss) with rapid high-dose IV injection

•      Hyperuricemia and gout precipitation

•      Glucose intolerance

•      Photosensitivity

NOT RECOMMENDED IN

•      Anuria or severe renal failure not responding to furosemide

•      Hepatic coma and precoma

•      Severe hypokalaemia or hyponatremia

•      Known hypersensitivity to furosemide or sulfonamides

DRUG INTERACTIONS

Combined use with aminoglycosides or cisplatin significantly increases ototoxicity risk. NSAIDs reduce the diuretic effect and increase the risk of renal impairment. ACE inhibitors and ARBs combined with furosemide can cause significant first-dose hypotension. Furosemide increases lithium plasma concentrations to toxic levels. Digoxin toxicity risk increases with furosemide-induced hypokalaemia.
