INDICATION

Fluderm Cap is a single-dose antifungal capsule primarily used for the treatment of uncomplicated vaginal candidiasis (vaginal thrush). A single oral dose of 150mg is curative in most cases of uncomplicated genital yeast infections. It may also be used for the treatment of oropharyngeal candidiasis and other superficial fungal infections caused by Candida species.

MECHANISM OF ACTION

Fluconazole inhibits the fungal cytochrome P450-dependent enzyme 14-alpha sterol demethylase, preventing the conversion of lanosterol to ergosterol — an essential structural component of the fungal cell membrane. This disrupts membrane integrity, alters permeability, and inhibits fungal growth. Fluconazole achieves high concentrations in vaginal secretions following oral administration, maintaining antifungal levels for up to 72 hours after a single dose.

DOSAGE AND ADMINISTRATION

•      Vaginal candidiasis: 150mg as a single oral dose

•      Oropharyngeal candidiasis: 200mg on Day 1, then 100mg once daily for 7 to 14 days

•      No food restrictions — take with or without food

•      Single dose is sufficient for uncomplicated vaginal candidiasis; no need for repeat

SIDE EFFECTS

Common:

•      Headache

•      Nausea, abdominal pain

•      Diarrhoea

•      Rash

Less common:

•      Hepatotoxicity (rare)

•      QT prolongation (at higher doses)

•      Stevens-Johnson syndrome (rare)

•      Hypersensitivity reactions

NOT RECOMMENDED IN

•      Known hypersensitivity to fluconazole or other azole antifungals

•      Concurrent use with terfenadine, pimozide, or cisapride (risk of fatal arrhythmias)

•      Use with caution in hepatic or renal impairment

DRUG INTERACTIONS

Fluconazole is a potent CYP2C9 and moderate CYP3A4 inhibitor. Increases warfarin levels (monitor INR), phenytoin, cyclosporine, tacrolimus, glibenclamide, and statin levels. A single 150mg dose has limited systemic effect compared to repeated dosing, but caution is still warranted in patients on interacting medications.
