INDICATION

Flow Dex 10% 1000ML is a sterile hypertonic dextrose solution for intravenous use. It is indicated for the treatment of hypoglycemia, provision of concentrated carbohydrate calories in patients requiring fluid restriction, supplemental caloric support in patients unable to maintain adequate oral intake, and as a vehicle for compatible IV medications.

MECHANISM OF ACTION

Dextrose 10% provides twice the caloric density of dextrose 5% (0.34 kcal/ml), delivering 340 kcal per 1000ml. Upon infusion, glucose enters cells via insulin-mediated GLUT transporters and undergoes glycolysis, generating ATP for energy metabolism. At 10% concentration, the solution is hypertonic relative to plasma, drawing intracellular fluid toward the vascular compartment and stimulating insulin secretion. It provides significant caloric support while administering smaller fluid volumes than D5W.

DOSAGE AND ADMINISTRATION

•      Administered intravenously under physician supervision

•      Rate and volume are individualized based on the patient's metabolic requirement, fluid balance, and clinical condition

•      For hypoglycemia correction: infuse at a rate sufficient to maintain blood glucose above 70 mg/dl

•      Monitor blood glucose regularly during administration

•      Infuse via a large peripheral vein or central venous access if prolonged therapy is required due to hypertonicity

•      Do not administer as a bolus without dilution

SIDE EFFECTS

Common:

•      Hyperglycemia, particularly in diabetic patients or with rapid infusion

•      Vein irritation or thrombophlebitis at the infusion site due to hypertonicity

Less common:

•      Fluid overload with excessive volumes

•      Hypokalemia due to glucose-stimulated insulin-mediated cellular potassium uptake

•      Hyponatremia with large volumes in susceptible patients

NOT RECOMMENDED IN

•      Uncontrolled diabetes mellitus without glycemic monitoring

•      Intracranial or intraspinal hemorrhage

•      Severe dehydration with hyperosmolality

•      Hypersensitivity to dextrose

DRUG INTERACTIONS

Corticosteroids may cause hyperglycemia when given concurrently with dextrose infusions. Insulin requirements increase during dextrose administration. Hypertonic dextrose may reduce the effectiveness of hypoglycemic agents. Magnesium salts are incompatible and should not be mixed directly.

STORAGE INSTRUCTIONS

Store at room temperature between 15 to 25 degrees Celsius. Protect from freezing. Do not use if solution is discolored or the container is damaged. Single use only.
