INDICATION

Exapro 5mg Tab is a selective serotonin reuptake inhibitor (SSRI) used for the treatment of major depressive disorder, generalized anxiety disorder, social anxiety disorder, panic disorder, and obsessive-compulsive disorder. The 5mg dose is used as a starting dose in elderly patients, those with hepatic impairment, or patients who are sensitive to SSRI side effects, particularly nausea and anxiety in the first weeks of treatment.

MECHANISM OF ACTION

Escitalopram is the S-enantiomer of citalopram and is the most selective SSRI available. It binds with high affinity and selectivity to the serotonin transporter (SERT), blocking serotonin reuptake into presynaptic neurons. This increases serotonergic neurotransmission in limbic areas of the brain involved in mood regulation, anxiety, and obsessive thought patterns. Unlike older SSRIs, escitalopram has minimal interaction with other receptors, contributing to its favorable tolerability profile.

DOSAGE AND ADMINISTRATION

•      Elderly and hepatically impaired patients: 5mg once daily; do not exceed 10mg per day

•      Adults (standard): start at 10mg once daily; may increase to 20mg after at least one week

•      Take once daily, in the morning or evening, with or without food

•      Full antidepressant effect typically takes 2 to 4 weeks

•      Do not stop abruptly; taper gradually to avoid discontinuation syndrome

SIDE EFFECTS

Common:

•      Nausea, dry mouth

•      Headache, dizziness

•      Insomnia or somnolence

•      Increased sweating

Less common:

•      QT prolongation at higher doses

•      Hyponatremia in elderly

•      Serotonin syndrome with serotonergic co-medications

•      Sexual dysfunction (delayed ejaculation, anorgasmia)

NOT RECOMMENDED IN

•      Concurrent MAOI use (14-day washout required)

•      Known QT prolongation or congenital long QT syndrome

•      Concurrent use with pimozide or linezolid

•      Known hypersensitivity to escitalopram or citalopram

DRUG INTERACTIONS

MAOIs and linezolid must not be combined — risk of fatal serotonin syndrome. QT-prolonging drugs (antipsychotics, antiarrhythmics) increase cardiac risk. NSAIDs and antiplatelet agents increase bleeding risk. Escitalopram is a mild CYP2D6 inhibitor; monitor drugs metabolized by this enzyme.
