INDICATION

Erli Plus 12.5/1000mg Tab is a higher-dose metformin formulation of the empagliflozin-metformin combination for adults with type 2 diabetes requiring optimized glycemic control. The 1000mg metformin component is appropriate for patients who are already established on, and tolerating, higher metformin doses.

MECHANISM OF ACTION

Same as Erli Plus 12.5/500mg. The higher metformin dose provides greater suppression of hepatic glucose production and improved insulin sensitivity, complemented by empagliflozin's renal glucose excretion mechanism.

DOSAGE AND ADMINISTRATION

•      1 tablet twice daily with meals

•      For patients established on metformin 1000mg

•      Monitor eGFR — do not use if below 45ml/min/1.73m2

•      Hold 48 hours before iodinated contrast procedures

SIDE EFFECTS

Common:

•      Nausea, diarrhoea, abdominal discomfort from higher metformin dose

•      Genital and urinary infections

•      Polyuria

Less common:

•      Diabetic ketoacidosis

•      Lactic acidosis

•      Volume depletion

NOT RECOMMENDED IN

•      eGFR below 45ml/min/1.73m2

•      Type 1 diabetes, DKA

•      Severe hepatic disease

DRUG INTERACTIONS

Insulin and sulphonylureas increase hypoglycemia risk. Diuretics increase volume depletion risk. Alcohol increases lactic acidosis risk from metformin.
