INDICATION

Epliron 50mg Tab is a selective mineralocorticoid receptor antagonist (MRA) used in the treatment of heart failure following acute myocardial infarction to reduce the risk of cardiovascular death and hospitalization. It is also indicated for the treatment of primary hyperaldosteronism and as add-on therapy in resistant hypertension. Unlike spironolactone, eplerenone has higher selectivity for the aldosterone receptor and fewer hormonal side effects.

MECHANISM OF ACTION

Eplerenone selectively blocks aldosterone from binding to mineralocorticoid receptors in the kidney, heart, blood vessels, and brain. Blockade of renal mineralocorticoid receptors promotes sodium and water excretion while retaining potassium, reducing blood pressure and cardiac preload. In the heart, it prevents the fibrotic and inflammatory effects of excessive aldosterone on cardiac muscle, reducing remodeling after myocardial infarction and slowing heart failure progression.

DOSAGE AND ADMINISTRATION

•      Heart failure post-MI: 25mg once daily initially; increase to 50mg once daily within 4 weeks if tolerated

•      Hypertension: 50mg once daily; may increase to 50mg twice daily

•      Monitor serum potassium and renal function before and during treatment

•      Reduce dose or discontinue if serum potassium exceeds 5.5 mmol/L

•      Take with or without food

SIDE EFFECTS

Common:

•      Hyperkalemia (high potassium) — most important adverse effect

•      Dizziness, headache

•      Diarrhoea, nausea

•      Fatigue

Less common:

•      Hypotension

•      Elevated serum creatinine

•      Gynecomastia and sexual dysfunction (much less frequent than with spironolactone)

•      Hyponatremia

NOT RECOMMENDED IN

•      Serum potassium above 5.0 mmol/L at initiation

•      eGFR below 30ml/min/1.73m2

•      Severe hepatic impairment

•      Concurrent use with potassium-sparing diuretics or potassium supplements

•      Concurrent use with strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole)

DRUG INTERACTIONS

Strong CYP3A4 inhibitors (ketoconazole, itraconazole, ritonavir, clarithromycin) significantly increase eplerenone plasma levels; concurrent use is contraindicated. ACE inhibitors and ARBs increase the risk of hyperkalemia when combined with eplerenone; monitor potassium closely. NSAIDs may reduce the antihypertensive effect and increase the risk of hyperkalemia and renal impairment.
