INDICATION

Empoli Plus 12.5/500mg Tab is a combination antidiabetic used for type 2 diabetes management when empagliflozin and metformin are both required. It is indicated as an adjunct to diet and exercise in adults with type 2 diabetes not adequately controlled on monotherapy. It also provides cardiovascular and renal benefits in high-risk patients.

MECHANISM OF ACTION

Empagliflozin inhibits SGLT2 in the renal proximal tubule, increasing urinary glucose excretion and reducing blood glucose without stimulating insulin. Metformin activates AMPK to suppress hepatic gluconeogenesis and improves peripheral insulin sensitivity. Together they produce additive glycemic lowering with low hypoglycemia risk and beneficial effects on weight and blood pressure.

DOSAGE AND ADMINISTRATION

•      1 tablet twice daily with meals

•      Individualize based on prior antidiabetic regimen

•      Swallow whole with water

•      Monitor renal function — hold if eGFR drops below 45ml/min/1.73m2

SIDE EFFECTS

Common:

•      Genital and urinary tract infections

•      Nausea, diarrhoea, abdominal discomfort

•      Increased urination

Less common:

•      Diabetic ketoacidosis

•      Volume depletion, hypotension

•      Lactic acidosis (rare)

NOT RECOMMENDED IN

•      eGFR below 45ml/min/1.73m2

•      Type 1 diabetes, DKA

•      Severe hepatic impairment

•      Pregnancy

DRUG INTERACTIONS

Same drug class interactions as other empagliflozin-metformin combinations. Monitor for hypoglycemia with insulin or sulphonylureas, and volume depletion with diuretics.
