INDICATION
Dulan 60mg Cap is the standard maintenance dose of duloxetine for major depressive disorder, generalized anxiety disorder, diabetic peripheral neuropathic pain, fibromyalgia, and chronic musculoskeletal pain including low back pain and osteoarthritis pain. This is the most studied and recommended therapeutic dose across all approved indications.
MECHANISM OF ACTION
At 60mg once daily, duloxetine achieves balanced, significant inhibition of serotonin and norepinephrine reuptake transporters, producing therapeutic antidepressant, anxiolytic, and analgesic effects. The dual-action mechanism addresses both the emotional and physical components of conditions such as depression with somatic pain and fibromyalgia.
DOSAGE AND ADMINISTRATION
•     Depression and GAD: 60mg once daily as maintenance
•     Diabetic neuropathic pain: 60mg once daily
•     Fibromyalgia and chronic musculoskeletal pain: 60mg once daily
•     Take with or without food; swallow whole
•     Do not stop abruptly; taper under medical supervision
SIDE EFFECTS
Common:
•     Nausea, dry mouth, constipation
•     Headache, dizziness, fatigue
•     Hyperhidrosis (excessive sweating)
•     Decreased libido, sexual dysfunction
Less common:
•     Hepatotoxicity
•     Serotonin syndrome
•     Elevated blood pressure
•     Hyponatremia in elderly patients
NOT RECOMMENDED IN
•     MAOIs within 14 days
•     Severe renal or hepatic impairment
•     Uncontrolled narrow-angle glaucoma
•     Known hypersensitivity to duloxetine
DRUG INTERACTIONS
At 60mg, CYP2D6 inhibition is clinically significant; monitor drugs metabolized by this enzyme. Avoid fluvoxamine, ciprofloxacin (increase duloxetine levels via CYP1A2). NSAIDs and anticoagulants increase bleeding risk.
