INDICATION

Dulan 30mg Cap is a serotonin-norepinephrine reuptake inhibitor (SNRI) used for the treatment of major depressive disorder, generalized anxiety disorder, diabetic peripheral neuropathic pain, fibromyalgia, and chronic musculoskeletal pain. The 30mg dose is used as the initiation dose or as a maintenance dose in elderly patients and those with hepatic impairment who cannot tolerate 60mg.

MECHANISM OF ACTION

Duloxetine inhibits the reuptake of both serotonin and norepinephrine with balanced dual activity in the central nervous system. By increasing synaptic availability of both neurotransmitters, it addresses depressive and anxious symptoms while also modulating descending pain inhibitory pathways in the spinal cord — making it effective for both mood disorders and neuropathic or musculoskeletal pain conditions.

DOSAGE AND ADMINISTRATION

•      Depression and GAD: 30mg once daily initially; increase to 60mg after one week if tolerated

•      Neuropathic pain: 30mg once daily; may increase to 60mg once daily

•      Elderly or hepatic impairment: remain on 30mg once daily

•      Take with or without food; swallow capsule whole, do not open

•      Taper dose gradually before stopping to avoid discontinuation syndrome

SIDE EFFECTS

Common:

•      Nausea, dry mouth

•      Headache, dizziness

•      Fatigue, constipation

•      Decreased appetite

Less common:

•      Increased blood pressure

•      Liver enzyme elevation

•      Hyponatremia in elderly

•      Serotonin syndrome with other serotonergic agents

NOT RECOMMENDED IN

•      Concurrent MAOI use or within 14 days of stopping an MAOI

•      Severe renal impairment (eGFR below 30ml/min)

•      Significant hepatic impairment

•      Uncontrolled narrow-angle glaucoma

DRUG INTERACTIONS

Duloxetine is a moderate CYP2D6 inhibitor. Do not combine with MAOIs, linezolid, or IV methylene blue. Avoid concurrent fluvoxamine or ciprofloxacin. Monitor warfarin INR and bleeding risk with NSAIDs.
