INDICATION

Dexavis 60mg Cap is used for the healing of all grades of erosive esophagitis in adults. The 60mg dose provides more complete acid suppression over 24 hours, making it appropriate for the acute healing phase in patients with moderate to severe erosive esophagitis (Los Angeles grades C and D) who require stronger acid inhibition.

MECHANISM OF ACTION

Same dual delayed-release mechanism as Dexavis 30mg. At 60mg, dexlansoprazole delivers a higher drug load across its two absorption pulses, achieving greater and more sustained inhibition of the H+/K+-ATPase proton pump in gastric parietal cells. The higher dose maintains intragastric pH above 4 for a significantly longer portion of the 24-hour period compared to 30mg, creating optimal conditions for mucosal healing in more severe erosive esophagitis. The DDR technology ensures consistent absorption regardless of meal timing.

DOSAGE AND ADMINISTRATION

•      Healing of erosive esophagitis (all grades): 60mg once daily for up to 8 weeks

•      After healing is confirmed, step down to 30mg once daily for maintenance

•      Can be taken at any time of day, with or without food

•      No dose adjustment required in mild to moderate hepatic impairment

•      Reduce dose in severe hepatic impairment — do not exceed 30mg once daily

SIDE EFFECTS

Common:

•      Diarrhoea, nausea, abdominal pain

•      Headache

•      Upper respiratory tract infections

•      Flatulence, vomiting

Less common:

•      Hypomagnesaemia with prolonged therapy

•      Vitamin B12 malabsorption with long-term use

•      Clostridium difficile-associated diarrhoea

•      Osteoporosis-related bone fractures with prolonged high-dose use

•      Acute tubulointerstitial nephritis (rare)

NOT RECOMMENDED IN

•      Known hypersensitivity to dexlansoprazole, lansoprazole, or other PPIs

•      Concurrent rilpivirine-containing antiretroviral regimens

•      Severe hepatic impairment — limit to 30mg if PPI therapy is essential

•      Avoid long-term use at 60mg without periodic clinical reassessment

DRUG INTERACTIONS

Same interaction profile as Dexavis 30mg. At the 60mg dose, reduction of ketoconazole, itraconazole, atazanavir, and iron salt absorption is more pronounced. Monitor digoxin and tacrolimus levels closely. CYP2C19 inhibitors (fluvoxamine, voriconazole) may increase dexlansoprazole plasma concentrations.
