INDICATION

Dexavis 30mg Cap is a proton pump inhibitor (PPI) with a dual delayed-release formulation used for the maintenance of healed erosive esophagitis, treatment of symptomatic non-erosive gastroesophageal reflux disease (GERD), and heartburn relief. The 30mg dose is used for maintenance therapy and for mild to moderate GERD symptoms.

MECHANISM OF ACTION

Dexlansoprazole is the R-enantiomer of lansoprazole. It is a prodrug that is activated in the acidic environment of the gastric parietal cell secretory canaliculi, where it covalently binds to and irreversibly inhibits the H+/K+-ATPase proton pump — the final enzyme in gastric acid secretion. Its unique dual delayed-release (DDR) technology delivers the drug in two separate pH-dependent pulses from the capsule: the first granule population dissolves at pH 5.5 in the proximal small intestine, and the second at pH 6.75 further along the intestine. This produces two distinct absorption peaks, extending the duration of acid suppression beyond 24 hours with a single daily dose — an advantage over conventional single-release PPIs.

DOSAGE AND ADMINISTRATION

•      Maintenance of healed erosive esophagitis: 30mg once daily

•      Symptomatic non-erosive GERD: 30mg once daily for 4 weeks

•      Heartburn relief: 30mg once daily

•      Can be taken at any time of day, with or without food — unlike conventional PPIs, no requirement to take before meals

•      Capsule may be opened and granules sprinkled on one tablespoon of applesauce for patients who cannot swallow; swallow immediately without chewing

•      Do not crush or chew the granules

SIDE EFFECTS

Common:

•      Diarrhoea, abdominal pain, nausea

•      Upper respiratory tract infection, nasopharyngitis

•      Headache, dizziness

•      Flatulence, vomiting

Less common:

•      Hypomagnesaemia with prolonged use — monitor magnesium in long-term therapy

•      Vitamin B12 deficiency with long-term use

•      Clostridium difficile-associated diarrhoea

•      Bone fracture risk with prolonged high-dose use

•      Fundic gland polyps with long-term use

NOT RECOMMENDED IN

•      Known hypersensitivity to dexlansoprazole, lansoprazole, or any other PPI

•      Concurrent use with rilpivirine-containing HIV regimens (significantly reduces rilpivirine absorption)

•      Use with caution in patients with severe hepatic impairment — dose reduction may be needed

•      Not recommended for self-treatment of symptoms without proper diagnosis

DRUG INTERACTIONS

Dexlansoprazole raises intragastric pH and reduces the absorption of drugs that require an acidic environment, including ketoconazole, itraconazole, iron salts, erlotinib, and atazanavir. It is metabolized by CYP2C19 and CYP3A4; CYP2C19 inhibitors (fluvoxamine, voriconazole) may increase dexlansoprazole exposure. It can increase plasma levels of methotrexate, digoxin, and tacrolimus. Clopidogrel antiplatelet efficacy may be modestly reduced via CYP2C19 competition, though the interaction is less pronounced than with omeprazole.
