INDICATION

Denla XR 50mg Tab is a serotonin-norepinephrine reuptake inhibitor (SNRI) approved for the treatment of major depressive disorder (MDD) in adults. It is the active metabolite of venlafaxine and provides consistent therapeutic levels with once-daily dosing due to its extended-release formulation.

MECHANISM OF ACTION

Desvenlafaxine is a potent inhibitor of serotonin and norepinephrine reuptake transporters (SERT and NET). By blocking reuptake, it increases the availability of serotonin and norepinephrine in the synaptic cleft, enhancing both monoaminergic systems involved in mood regulation. Unlike venlafaxine, desvenlafaxine is not significantly metabolized by CYP2D6, which reduces interindividual pharmacokinetic variability.

DOSAGE AND ADMINISTRATION

•      Recommended dose: 50mg once daily at the same time each day

•      No additional benefit demonstrated with doses above 50mg/day in most patients

•      May be taken with or without food

•      Do not discontinue abruptly; taper the dose gradually under medical supervision

SIDE EFFECTS

Common:

•      Nausea (most common, especially in the first 2 weeks)

•      Headache, dizziness

•      Dry mouth, constipation

•      Insomnia, excessive sweating

Less common:

•      Increased blood pressure and heart rate (monitor regularly)

•      Hyponatremia, especially in elderly patients

•      Serotonin syndrome when combined with other serotonergic agents

•      Suicidal ideation in young adults during initial treatment

NOT RECOMMENDED IN

•      Concurrent or recent use of MAOIs (allow 14 days after stopping an MAOI)

•      Uncontrolled narrow-angle glaucoma

•      Known hypersensitivity to desvenlafaxine or venlafaxine

•      Use with caution in patients with hypertension, cardiovascular disease, or seizure disorder

DRUG INTERACTIONS

Desvenlafaxine should not be combined with MAOIs, linezolid, or IV methylene blue due to the risk of serotonin syndrome. It can increase the anticoagulant effect of warfarin; monitor INR. Concurrent use with NSAIDs, aspirin, or antiplatelet agents increases the risk of gastrointestinal bleeding.
