INDICATION

Clotrim Cream 20gm is a topical imidazole antifungal indicated for the treatment of dermatophyte infections (tinea corporis, tinea cruris, tinea pedis), cutaneous candidiasis (candidal intertrigo and paronychia), pityriasis versicolor, and erythrasma caused by Corynebacterium minutissimum. It is used for superficial fungal skin infections presenting as itching, scaling, redness, and skin discomfort.

MECHANISM OF ACTION

Clotrimazole is a broad-spectrum imidazole antifungal that inhibits the fungal cytochrome P450 enzyme lanosterol 14-alpha-demethylase, blocking the conversion of lanosterol to ergosterol, the principal sterol of the fungal cell membrane. Depletion of ergosterol and accumulation of toxic sterols in the cell membrane disrupts membrane integrity and permeability, causing leakage of essential intracellular components and fungal cell death. Clotrimazole is fungistatic at low concentrations and fungicidal at higher concentrations and has activity against dermatophytes (Trichophyton, Microsporum, Epidermophyton), Candida species, and Malassezia furfur.

DOSAGE AND ADMINISTRATION

•      Apply a thin layer to the affected skin area and surrounding region twice daily (morning and evening)

•      Treatment duration: 2 to 4 weeks for most tinea infections; 4 weeks for tinea pedis

•      Keep the area clean and dry; wear breathable footwear and loose clothing

•      Always complete the full treatment course even if symptoms improve to prevent relapse

SIDE EFFECTS

Common:

•      Mild burning or stinging on application (usually transient)

•      Skin erythema at the application site

•      Mild itching during the first few days of use

•      Skin dryness or flaking

Less common:

•      Allergic contact dermatitis in individuals sensitive to clotrimazole or imidazole antifungals

•      Skin blistering in hypersensitive individuals

•      Oedema at the application site (rare)

•      Urticaria in very sensitive individuals

NOT RECOMMENDED IN

•      Known hypersensitivity to clotrimazole or other imidazole antifungals

•      Ophthalmic use (separate ophthalmic formulations exist for eye infections)

•      Not for deep or systemic fungal infections (topical only)

•      If no improvement after 4 weeks of treatment, reassess the diagnosis with a dermatologist

DRUG INTERACTIONS
