INDICATION

Canter 250mg Tab is a macrolide antibiotic used for the treatment of community-acquired pneumonia, acute exacerbations of chronic bronchitis, sinusitis, pharyngitis, tonsillitis, skin and soft tissue infections, and Helicobacter pylori eradication as part of triple therapy. It is also used in combination regimens for Mycobacterium avium complex (MAC) prophylaxis in HIV-positive patients.

MECHANISM OF ACTION

Clarithromycin binds reversibly to the 50S ribosomal subunit of susceptible bacteria, inhibiting the translocation step of protein synthesis. This bacteriostatic action prevents bacterial growth. Clarithromycin is also converted to its active 14-hydroxyclarithromycin metabolite in vivo, which has additional antibacterial activity. It demonstrates excellent tissue penetration, achieving concentrations in lung, sinus, and tonsillar tissue several times higher than serum levels.

DOSAGE AND ADMINISTRATION

•      Adults: 250mg twice daily for 7 to 14 days depending on the infection

•      H. pylori eradication: 500mg twice daily (higher strength) with a PPI and amoxicillin

•      Community-acquired pneumonia: 500mg twice daily for 7 to 14 days

•      Tablets can be taken with or without food; taking with food reduces gastric side effects

•      Dose reduction required in patients with severe renal impairment

SIDE EFFECTS

Common:

•      Nausea, vomiting, abdominal discomfort

•      Altered taste (metallic taste), dry mouth

•      Headache, dizziness

•      Diarrhoea

Less common:

•      QT prolongation and risk of serious cardiac arrhythmias

•      Cholestatic jaundice and hepatotoxicity

•      Clostridium difficile-associated colitis

•      Hearing loss or tinnitus (reversible) with high doses

NOT RECOMMENDED IN

•      Known hypersensitivity to clarithromycin or other macrolide antibiotics

•      Concomitant use with colchicine in patients with renal or hepatic impairment

•      Concomitant use with pimozide, ergotamine, or dihydroergotamine

•      Patients with known QT prolongation or hypokalaemia

DRUG INTERACTIONS

Clarithromycin is a potent CYP3A4 inhibitor and P-glycoprotein inhibitor. It can significantly increase plasma levels of statins (simvastatin, lovastatin), increasing the risk of myopathy and rhabdomyolysis. Clarithromycin increases cyclosporine, tacrolimus, carbamazepine, warfarin, and digoxin levels. Concurrent use with colchicine is potentially fatal in patients with renal impairment. Rifampicin and rifabutin reduce clarithromycin levels.
