INDICATION

Brim Scabies Lotion 100ml is a topical insecticide and acaricide indicated for the treatment of scabies caused by the mite Sarcoptes scabiei in adults and children 2 months of age and older. It is the first-line recommended treatment for scabies globally due to its high efficacy, safety profile, and single-application cure rates. It is also indicated for the treatment of head lice (pediculosis capitis) and pubic lice as a second indication, providing effective elimination of lice and their eggs (nits) with a single application.

MECHANISM OF ACTION

Permethrin is a synthetic pyrethroid insecticide and acaricide that acts on the nervous system of mites and lice by binding to voltage-gated sodium channels in nerve cell membranes and preventing their closure after activation. This prolongs sodium influx, causing persistent nerve depolarization, paralysis of the mite or louse, and death. Permethrin has selective toxicity for arthropods over mammals because mammalian sodium channels recover much more rapidly from permethrin binding and mammals rapidly metabolize permethrin through hepatic ester hydrolysis to inactive compounds. At 5% concentration, permethrin penetrates the scabies burrow and kills both adult mites and eggs.

DOSAGE AND ADMINISTRATION

•      Scabies: massage cream or lotion thoroughly into all skin from the neck down to the soles of the feet, including between fingers and toes, under nails, and in skin folds

•      Leave on for 8 to 14 hours (overnight application is preferred), then wash off thoroughly with soap and water

•      A second application after 7 to 14 days is recommended to eliminate any mites hatching from eggs that survived the first treatment

•      All household members and close contacts should be treated simultaneously regardless of symptoms; wash all clothing, bed linen, and towels in hot water on the same day

SIDE EFFECTS

Common:

•      Mild burning, stinging, or tingling at the application site (transient; expected)

•      Skin pruritus and redness (may be caused by the scabies immune reaction rather than the product)

•      Skin numbness or paraesthesia at the application site

•      Mild skin rash or erythema

Less common:

•      Oedema and skin swelling in sensitive individuals

•      Allergic contact dermatitis in individuals sensitive to pyrethroids or chrysanthemum

•      Exacerbation of eczema in patients with pre-existing atopic dermatitis

•      Eye irritation if product contacts the eyes (rinse immediately with water)

NOT RECOMMENDED IN

•      Known hypersensitivity to permethrin, pyrethroids, or chrysanthemum (Asteraceae family)

•      Avoid contact with eyes, mouth, and mucous membranes

•      Infants below 2 months of age without specialist guidance

•      Do not apply to inflamed, raw, or broken skin without medical advice

DRUG INTERACTIONS

Topical permethrin at therapeutic dermatological doses has no clinically significant pharmacokinetic drug interactions with common systemic medications due to its minimal systemic absorption and rapid hepatic metabolism. Concurrent use of other topical preparations on the same skin area may reduce permethrin contact time and efficacy; apply permethrin to clean, dry skin without other topical products.
