INDICATION

Bonviva 150mg Tab is a bisphosphonate used for the treatment and prevention of postmenopausal osteoporosis. Monthly dosing reduces the burden of daily pill-taking for patients with osteoporosis, improving adherence. It reduces the risk of vertebral fractures and may also reduce hip and non-vertebral fractures when used as part of a comprehensive osteoporosis management programme that includes calcium and vitamin D supplementation.

MECHANISM OF ACTION

Ibandronate is a nitrogen-containing bisphosphonate that binds tightly to hydroxyapatite in bone mineral at sites of active bone remodeling. It inhibits osteoclast-mediated bone resorption by suppressing the mevalonate pathway, blocking farnesyl pyrophosphate synthase, and disrupting osteoclast function and survival. This reduces excessive bone turnover, increases bone mineral density, and decreases the rate of bone loss, reducing fracture risk over time.

DOSAGE AND ADMINISTRATION

•      One 150mg tablet taken orally once per month on the same date each month

•      Take first thing in the morning on an empty stomach, at least 30 to 60 minutes before any food, drink, or other medications

•      Swallow with a full glass of plain water (180 to 240ml)

•      Remain upright (sitting or standing) for at least 60 minutes after taking the tablet

•      Do not take with mineral water, coffee, tea, milk, or juice

SIDE EFFECTS

Common:

•      Abdominal pain, dyspepsia, heartburn

•      Nausea, constipation

•      Musculoskeletal pain (bone, joint, muscle aches)

•      Headache, dizziness

Less common:

•      Esophageal reactions including ulceration and stricture if swallowed incorrectly

•      Osteonecrosis of the jaw, particularly in cancer patients on high-dose therapy

•      Atypical femoral fractures with very long-term use

•      Acute phase reactions (flu-like symptoms) after first dose

NOT RECOMMENDED IN

•      Hypocalcemia (must be corrected before starting therapy)

•      Abnormalities of the esophagus that delay esophageal emptying

•      Inability to sit or stand upright for at least 60 minutes after dosing

•      Known hypersensitivity to ibandronate or other bisphosphonates

•      Severe renal impairment (creatinine clearance below 30ml/min)

DRUG INTERACTIONS

Products containing calcium, antacids, and other oral medications can interfere with ibandronate absorption when taken simultaneously; administer at least 6 hours before or after ibandronate. NSAIDs and aspirin combined with bisphosphonates may increase the risk of gastrointestinal side effects. Aminoglycosides combined with bisphosphonates can cause additive hypocalcemia.
