INDICATION

Berica 90mg Tab is a selective COX-2 inhibitor prescribed for the relief of pain and inflammation in acute gout, osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and acute musculoskeletal pain. It is also indicated for the short-term treatment of postoperative dental pain. Its once-daily dosing offers convenience for patients requiring regular anti-inflammatory therapy.

MECHANISM OF ACTION

Etoricoxib selectively inhibits the cyclooxygenase-2 (COX-2) enzyme, which is responsible for the synthesis of prostaglandins at sites of inflammation. By blocking COX-2 without significantly affecting COX-1 (which protects the gastric mucosa and supports platelet function), etoricoxib reduces inflammation and pain while presenting a lower risk of gastrointestinal ulceration compared to non-selective NSAIDs. Its analgesic and anti-inflammatory effects are comparable to non-selective NSAIDs at therapeutic doses.

DOSAGE AND ADMINISTRATION

•      Acute gout: 120mg once daily for up to 8 days (not the 90mg strength)

•      Osteoarthritis and musculoskeletal pain: 60mg once daily (lower strength preferred)

•      Rheumatoid arthritis/ankylosing spondylitis: 90mg once daily

•      Postoperative dental pain: 90mg once daily for up to 3 days

•      Take with or without food; avoid in patients with cardiovascular risk

SIDE EFFECTS

Common:

•      Headache, dizziness

•      Oedema (swelling), elevated blood pressure

•      Gastrointestinal discomfort, dyspepsia

•      Elevated liver enzymes (transaminases)

Less common:

•      Increased risk of serious cardiovascular events including heart attack and stroke

•      Severe skin reactions (Stevens-Johnson syndrome)

•      Renal impairment, fluid retention

•      Hypersensitivity reactions in aspirin-sensitive patients

NOT RECOMMENDED IN

•      Established ischemic heart disease, peripheral arterial disease, or cerebrovascular disease

•      Active peptic ulceration or gastrointestinal bleeding

•      Severe hepatic or renal impairment

•      Pregnancy (third trimester) and breastfeeding

•      Known hypersensitivity to etoricoxib or aspirin-induced bronchospasm

DRUG INTERACTIONS

Etoricoxib can reduce the antihypertensive and diuretic effects of ACE inhibitors, ARBs, and thiazide diuretics. Concurrent use with warfarin can increase INR and risk of bleeding. Co-administration with lithium may increase lithium plasma concentrations. Rifampicin reduces etoricoxib plasma levels by approximately 65%, decreasing its efficacy. Antacids have no clinically significant effect on etoricoxib absorption.
