INDICATION

Beltapro Oint 30gm is a very potent topical corticosteroid indicated for the treatment of severe, resistant, inflammatory, and pruritic skin conditions that do not respond adequately to less potent corticosteroid preparations. It is used in psoriasis (excluding widespread plaque psoriasis), resistant eczema and dermatitis, lichen planus, lichen simplex chronicus, discoid lupus erythematosus, and other steroid-responsive dermatoses requiring a high-potency corticosteroid.

MECHANISM OF ACTION

Clobetasol propionate is a superpotent (Class I) synthetic fluorinated corticosteroid that binds to intracellular glucocorticoid receptors with very high affinity and potency. It modulates the transcription of anti-inflammatory genes, suppressing the production of pro-inflammatory cytokines, prostaglandins, leukotrienes, and histamine. The ointment base provides greater occlusion and penetration than cream or lotion formulations, enhancing percutaneous absorption and increasing the anti-inflammatory and antipruritic effect in thick, hyperkeratotic, or recalcitrant skin lesions. It rapidly reduces oedema, erythema, and pruritus in severe inflammatory skin diseases.

DOSAGE AND ADMINISTRATION

•      Apply a thin layer to the affected area twice daily for up to 2 weeks only

•      Do not use for longer than 2 consecutive weeks without medical reassessment

•      Limit application to the smallest effective area; avoid the face, groin, and axillae

•      Always follow the dosage and duration prescribed by your dermatologist; maximum weekly dose should not exceed 50g in adults

SIDE EFFECTS

Common:

•      Skin atrophy, thinning, and striae with prolonged or extensive use

•      Skin burning and stinging on application

•      Dry skin and folliculitis

•      Perioral or periorbital dermatitis with facial use

Less common:

•      Clinically significant hypothalamic-pituitary-adrenal axis suppression with extensive or prolonged use, particularly in children

•      Cushing's syndrome with excessive systemic absorption

•      Secondary bacterial, viral, or fungal skin infections

•      Telangiectasia and hypopigmentation at application sites

NOT RECOMMENDED IN

•      Rosacea, acne vulgaris, perioral dermatitis, and facial use

•      Infected skin without adequate antimicrobial cover

•      Children under 12 years (significantly higher systemic absorption risk)

•      Use on the face, armpits, groin, or under occlusion without specialist instruction

DRUG INTERACTIONS

Topical corticosteroids have minimal systemic drug interactions at standard doses. However, the very high potency of clobetasol means that systemic absorption is more clinically significant, especially with extensive or prolonged use, occlusive dressings, or use on skin with compromised barrier function. Systemic corticosteroid-drug interactions may apply with significant absorption.
