INDICATION

Azobar 500mg is a higher-strength macrolide antibiotic indicated for community-acquired pneumonia requiring hospitalization or in patients with more severe infections, prophylaxis of Mycobacterium avium complex (MAC) in HIV-infected patients, and as a day-1 loading dose in standard z-pack regimens for respiratory and soft tissue infections.

MECHANISM OF ACTION

Same mechanism as Azobar 250mg. The 500mg dose achieves higher plasma and tissue concentrations, providing more rapid bactericidal activity against susceptible organisms. It is the standard day-1 loading dose in z-pack therapy and the single-dose daily regimen for moderate to severe community-acquired pneumonia requiring hospital-level care.

DOSAGE AND ADMINISTRATION

•      Community-acquired pneumonia: 500mg once daily for 3 to 7 days

•      Z-pack initiation: 500mg on day 1, followed by 250mg on days 2 to 5

•      MAC prophylaxis in HIV: 1200mg once weekly (usually with other antimycobacterial)

•      Always follow the dosage and duration prescribed by your doctor; complete the full course

SIDE EFFECTS

Common:

•      Nausea, vomiting, and diarrhoea (more likely at 500mg)

•      Abdominal pain

•      Headache

•      QT prolongation at higher doses

Less common:

•      Cardiac arrhythmias in susceptible patients

•      Clostridium difficile colitis

•      Hepatotoxicity

•      Hypersensitivity reactions

NOT RECOMMENDED IN

•      Known hypersensitivity to macrolide antibiotics

•      QT prolongation risk or concurrent QT-prolonging drugs

•      Severe hepatic impairment

•      History of cholestatic jaundice with prior azithromycin

DRUG INTERACTIONS

Same as Azobar 250mg. QT-prolonging drugs must be avoided. Monitor INR with warfarin. Cyclosporine levels increase. Antacids reduce absorption rate.
