INDICATION

Acefyl Resp 125 Syrup is a xanthine bronchodilator indicated for the relief of bronchospasm in bronchial asthma, chronic bronchitis, and other reversible obstructive airway diseases in adults and children. It is used to reduce wheezing, breathlessness, and chest tightness associated with acute and chronic airway obstruction. The syrup formulation makes it particularly suitable for pediatric patients and adults who have difficulty swallowing solid dosage forms. It is commonly prescribed as an adjunctive bronchodilator alongside inhaled therapies in chronic respiratory disease management.

MECHANISM OF ACTION

Acefylline piperazine is a piperazine salt of theophylline that combines the bronchodilatory properties of theophylline with better gastrointestinal tolerability than plain theophylline. It inhibits phosphodiesterase enzymes in bronchial smooth muscle, increasing intracellular cyclic AMP, which causes smooth muscle relaxation and bronchodilation. The piperazine moiety improves water solubility and reduces gastrointestinal irritation. Acefylline also has mild anti-inflammatory properties, reduces mucociliary dysfunction, and improves diaphragmatic contractility in patients with COPD.

DOSAGE AND ADMINISTRATION

•      Adults: 200 to 400mg (8 to 16ml) two to three times daily

•      Children 6 to 12 years: 100 to 200mg (4 to 8ml) two to three times daily

•      Children 2 to 6 years: 50 to 100mg (2 to 4ml) two to three times daily

•      Take with or after meals to reduce gastrointestinal irritation; always follow the dosage prescribed by your doctor

SIDE EFFECTS

Common:

•      Nausea and vomiting

•      Headache and dizziness

•      Palpitations and tachycardia

•      Restlessness and insomnia

Less common:

•      Seizures at toxic plasma levels

•      Cardiac arrhythmias with excessive dosing

•      Hypotension

•      Gastrointestinal cramps and diarrhoea

•      Known hypersensitivity to theophylline or xanthine derivatives

•      Acute myocardial infarction

•      Peptic ulcer disease (xanthine’s can increase gastric acid secretion)

•      Caution in epilepsy, hyperthyroidism, and severe hepatic impairment

DRUG INTERACTIONS

Erythromycin, cimetidine, ciprofloxacin, and allopurinol inhibit theophylline metabolism, increasing plasma levels and toxicity risk; monitor closely. Rifampicin, carbamazepine, and phenobarbitone induce xanthine metabolism, reducing efficacy. Caffeine has additive CNS and cardiovascular stimulant effects; advise patients to limit coffee and tea. Beta-blockers may antagonize the bronchodilatory effect. Phenytoin reduces theophylline levels through enzyme induction.
